摘要
目的探讨聚焦解决护理模式提高尿路造口患者自我护理和生活质量的效果。方法便利抽样选择2013年6月至2014年5月择期在全麻下行腹腔镜下全膀胱切除+回肠代膀胱患者40例作为研究对象,根据住院时间先后分为聚焦解决模式组20例及对照组20例,分别采用聚焦解决护理模式和常规护理;比较干预前后两组患者自我护理知识知晓情况、出院3个月后并发症的发生率以及世界卫生组织生活质量测定量表(WHO quality of life-100,WHOQOL-100)的得分情况。结果干预后聚焦组造口自我护理知识知晓率高于对照组(P<0.05),出院3个月并发症发生率低于对照组,差异均有统计学意义(均P<0.01);干预前两组患者WHOQOL-100评分比较,差异无统计学意义(P>0.05),干预后聚焦组明显高于对照组,差异有统计学意义(P<0.01)。结论聚焦解决护理模式发挥了尿路造口患者自我护理的主动性,不仅显著提高了尿路造口患者自护水平,降低了并发症的发生,同时提高了患者的生活质量和手术效果满意度,有助于促进手术成功及疾病康复,值得临床推广应用。
Objective To explore the practical effect of focusing solution nursing model on improving self-management of patients with urinary tract stoma.Methods By convenience sampling,40 patients with urinary tract stoma were equally divided into the focusing solution model group and the control group.The focusing solution model group received focusing solution model of nursing, and the control group received conventional nursing. The self-care knowledge level, the incidence of postoperative complications after 3 months of discharge and the score of WHOQOL-100 before and after intervention were compared be- tween two groups.Results After the intervention,the awareness rate of self-nursing knowledge of the fo- cusing solution model group was higher than that of the control group（P〈0.05）.The incidence of postoperative complications after 3 months of discharge in focusing solution model group was lower than the control group（P〈0.01）.There was no statistical significance of the score of WHO QOL-100 between two groups before intervention （P〉0.05） ,while the score of WHO QOL-100 in focusing solution model group was higher than the control group after intervention （ P〈0. 01）. Conclusion Focusing solution model of nursing takes initiative of self-care in patients with urinary tract stoma, it not only significantly improves the level of self-care and also reduces the incidence of complication,and in the mean time improves the pa- tients quality of life and satisfaction towards surgical effect, which can promote the success of operation and rehabilitation.It is worthy for clinical application.
