摘要
目的:探讨高血压病患者左室舒张功能不全及脉搏波传导速度的相关因素,并进一步探讨二者的相关性。方法:选取高血压病左室舒张功能不全组21例(C组)、高血压病无心力衰竭组21例(B组)以及健康对照组20例(A组),测量踝臂脉搏波传导速度(baPWV),E/E'等左室舒张功能相关指标,以及B型钠尿肽(BNP)、超敏C反应蛋白(hs-CRP)等生化指标。结果:与A组比较,B组和C组的baPWV均显著升高[(6.5±2.5)m/s︰(8.7±2.1)m/s、(12.7±4.2)m/s,P<0.05]。多元逐步线性回归分析显示,收缩压、BNP与baPWV呈独立正相关(β=0.300、0.376,P<0.05)。baPWV、脉压是左室舒张功能指标E/E'的独立影响因素(β=0.474、0.296,P<0.05),随着baPWV、脉压的增大,E/E'增大。结论:BNP与收缩压是baPWV的独立影响因素;baPWV与脉压是左室舒张功能指标E/E'的独立影响因素。随着baPWV的升高,E/E'升高,左室舒张功能减退。
Objective:To investigate the relevant factors of left ventricular diastolic dysfunction and pulse wave velocity(PWV)in hypertensive patients,then further discuss correlation between them.Method:baPWV,left ventricular diastolic function and other biochemical indexes such as BNP,hs-CRP and so on were measured among three groups,including group of 21 hypertensive patients with left ventricular diastolic dysfunction(group C),21 hypertensive patients without left ventricular dysfunction(group B)and 20 cases as normal control(group A).Result:Compared to group A,the baPWV in group B and group C increased significantly [(6.5±2.5)m/s vs.(8.7±2.1)m/s,(12.7±4.2)m/s,P〈0.05.Multiple stepwise linear regression analysis showed that systolic blood pressure and BNP were independently and positively associated with baPWV(β=0.300,0.376,P〈0.05).baPWV and pulse pressure were independent factors of E/E'(β=0.474,0.296,P〈0.05),with the increase of baPWV or pulse pressure,E/E'elevated.Conclusion:Both BNP and SBP are independent factors of baPWV.The independent factors of E/E'are baPWV and PP,with the increases of baPWV,E/E'increases,left ventricular diastolic function decreases.
