摘要
目的探讨腹腔镜下子宫内膜异位症生育指数对EMT合并不孕患者生育能力的评估价值。方法收集2011年1月至2014年1月收治的100例子宫内膜异位症合并不孕患者,对患者进行治疗,观察其效果及随访结果。结果腹腔镜术后36个月的累积妊娠率为45.0%,与EFT评分具有显著的正相关性(F=9.394,P〈0.05),与r-AFS分期没有显著的相关性(F=0.822,P〉0.05)。在EFT为0~4分、5~7分、8~10的患者中,术后36个月的累积妊娠率与术后6个月相比,差异有统计学意义(χ2=9.428、7.394、6.692,P〈0.05)。EFT评分在8~10分的患者其术后6个月、12个月、24个月、36个月时的累积妊娠率与0~4分、5~7分的患者相比,差异有统计学意义(χ2=7.383、6.395、5.321、6.285,P〈0.05)。在EFT评分为0~4分与5~7分的患者中,宫腔内人工授精和自然周期与促排卵周期相比累积妊娠较低,差异有统计学意义(χ2=4.501,P〈0.05)。结论腹腔镜下子宫内膜异位症生育指数能够评价子宫内膜异位症合并不孕患者的生育能力,可以通过EFT评分情况对患者采取积极措施,提高患者的妊娠率。
Objective To investigate the laparoscopic endometriosis fertility index in the fertility assessment of the EMT patient combined with infertility. Methods One hundred cases of EMT patient combined with infertility were selected from Jan. 2011 to Jan. 2014. The outcome of treatment and follow-up results were observed. Results The cumulative pregnancy rate of 36 months after laparoscopic surgery was45. 0%,which had positive correlation with EFT score( F = 9. 394,P 0. 05),while there was no significant correlation with r-AFS stage( F = 0. 822,P 0. 05). In patient with EFT score in group 0-4、5-7and 8-10,the cumulative pregnancy rate after 36 months was significantly higher than that after 6 months( χ2= 9. 428,7. 394,6. 692,P 0. 05). there was also significant difference in the cumulative pregnancy rate after 36 months between group 0-4、5-7 and 8-10( χ2= 9. 428,7. 394、6. 692,P 0. 05). The cumulative pregnancy rate was significantly different between 6,12,24,36 months after surgery( χ2= 7. 383,6. 395,5. 321,6. 285,P 0. 05) in EFT score of 8-10,while there was no different in group 0-4 or group 5-7. In patients with EFT score 0-4 or 5-7,the cumulative pregnancy rate was significantly lower after IUI or natural cycle than COH cycle( χ2= 4. 501,P 0. 05). Conclusions Laparoscopic endometriosis fertility index can effectively assess the fertility in EMT patients combined with infertility. The pregnancy rate can be increased after appropriate treatment according to the EFT score.
