右下后牙缺牙区改良骨劈开同期种植术1例报告 被引量:1

摘要 目的:评价后牙区行骨劈开术并同期植入种植体后的临床疗效。方法:右下后牙区45、46牙位牙列缺损患者,患者缺牙区牙槽嵴高度尚可(≥12mm),水平宽度不足,约为3-4mm。行改良骨劈开术,采取减张设计及器械撑开结合,顺利劈开12mm长,扩张约4mm后同期植入Strauman种植体两枚,间隙植入骨粉;骨愈合后上端采用固定修复方式。结果:术中未出现骨壁折断游离状况,种植体窝洞预备情况良好,患者疼痛,肿胀反应轻微,观察6个月后种植体与周围骨结合理想,未出现明显骨壁吸收情况,上部结构修复后功能和美观效果俱佳。结论:下颌后牙区在把握好适应症,做好术前评估的前提下,采用改良骨劈开术能有效增加水平骨宽度,为种植体同期植入提供了良好的空间和条件,修复获得了较满意的临床疗效。 Objective: To evaluate the clinical effects using the modified bone split with simultaneously im-planting in the mandibular molar region. Methods: The patient had lost the 45 and 46, the remaining height of the alveolar ridge in this region was equal to or greater than 12mm, horizontal width was defi-ciency about 3-4mm. The modified bone split method combined reducing the tension and opening by the instruments. After splitting 12mm in length and opening 4mm in width, inserting two implants (Straumann, Switzerland). The gap between the implant and bone was filled with bone meal. After the osseointegration had formed, using the fixed restorations to complete the therapy. Results: During the surgery, there was not any free bone fragments, the holes were well prepared. Pain and swelling were slight. After 6 months healing, the osseointegration was formed well. We had not observed obvious bone resorption. With the fixed restorations we achieved preferable function and aesthetic effect. Conclusion:Controlling the indication strictly and well preparing before the surgery, using the modified bone split method can acquire sufficient width of the alveolar bone which is benefit for the implant insertion. After restoration we can achieve satisfying clinical effects.
