摘要
目的:比较日间手术医院-社区延伸康复模式与住院手术全程康复模式对乳腺良性肿瘤的安全性。方法:选取本院2014年1月-2015年1月收治的200例乳腺良性肿瘤患者为研究对象,双盲法随机将其分为两组,各100例,对照组患者采取常规手术全程康复模式,观察组患者则行日间手术医院-社区延伸康复模式,比较两种康复模式的医疗安全性(伤口出血、感染、麻醉事件发生率、呼吸道感染)及患者满意度。结果:观察组患者伤口感染、呼吸道感染发生率分别为1.0%、0,与对照组的7.0%、5.0%比较,差异有统计学意义(P<0.05)。另外,观察组和对照组患者满意度的相关指标评分比较,包括医疗费用评分、服务态度评分、伤口愈合及护理水平评分,差异均有统计学意义(P<0.05)。结论:日间手术医院-社区延伸康复模式与住院手术全程康复模式医疗安全类似,且能明显降低医院感染率,提高患者满意度,值得临床推广。
Objective： To compare the safety of day surgery with hospital-community rehabilitation extension mode and inpatient surgery with whole-course rehabilitation mode in the treatment of benign breast tumor. Method： 200 patients with benign breast tumor from January 2014 to January 2015 in our hospital were selected as the research objects and divided into two groups by randomly double blind method with 100 eases in each group. The control group was treated with conventional operation whole-course rehabilitation mode, while, the observation group with day operation with hospital-community extension rehabilitation mode. The security （such as wound bleeding, infection, the incidence rate of anesthesia and respiratory tract infection ） and patients＇ satisfaction of the two groups were compared. Result： The rate of wound infection and respiratory tract infection of the observation group was 1.0% and 0 respectively, compared with 7.0% and 5.0% of the control group, the differences were significant （ P〈0.05 ） . In addition, patients＇ satisfaction score comparison of relevant indicators between the two groups, including medical expenses points, service attitude, wound healing points, and nursing level, satisfaction score, showed that the differences were statistically significant （ P〈0.05 ） . Conclusion： The medical safety of day surgery with hospital-community extension rehabilitation model is similar to that of the inpatient surgery with whole-course rehabilitation mode. The former can significantly reduces the rate of hospital infection, improve the patients＇ satisfaction and is worthy of clinical promotion.
