兔椎间失稳后弹性内固定软骨终板Ⅱ型胶原蛋白变化的实验研究 被引量:2

摘要 目的观察兔软骨终板在椎间失稳环境下,经弹性内固定重建椎间稳定,软骨终板Ⅱ型胶原蛋白的变化,为腰腿痛疾病的治疗提供新的思路和方法。方法选取36只6个月龄日本大耳白兔,雄雌不限,体质量(2.5±0.15)kg,随机分为对照组和实验组各18只。先将实验所用36只兔制作成L6/7椎间手术失稳模型;实验组在手术失稳2个月后用椎弓根螺丝钉和张力钢丝弹性内固定的方法进行L6/7的稳定性重建,术后2、4、6个月分别取2组兔椎间盘软骨终板(每个时点6只),用免疫组织化学的方法检测Ⅱ型胶原蛋白,用JD801图像分析系统进行图像分析,综合判断软骨终板的退行性变过程及机制。结果实验组手术进行椎间失稳张力钢丝弹性内固定稳定性重建后,2、4、6个月短期内椎间软骨终板厚度较对照组明显变厚,软骨细胞显著增加。实验组Ⅱ型胶原蛋白灰度术后2~6个月逐渐下降,实验组Ⅱ型胶原蛋白灰度术后2~6个月均低于对照组(P〈0.01);实验组Ⅱ型胶原蛋白光密度术后2~6个月逐渐升高,实验组Ⅱ型胶原蛋白光密度术后2~6个月均高于对照组(P〈0.01)。结论弹性内固定能有效阻止软骨终板Ⅱ型胶原蛋白的退行性变;而且有部分恢复的征象,为进一步进行其他治疗创造了必要条件。 Objective By studying mechanism of the rabbit cartilage endplate collagen typeⅡ change,adopting the method of dynamic internal fixation to reconstruction of intervertebral stability after the intervertebral destabilization,providing new ideas and methods for the clinical treatment of related diseases.Methods The study chose 36 white big ear rabbits of Japan,body mass(2.5±0.15)kg,female and male unrestricted.The rabbits were randomly divided into control group and experimental group,each group 18 rabbits.Firstly,all rabbits were destabilizated by operated on L6/7.Only the experimental group ran to reconstruct the intervertebral stabilization of L6/7two months later,using the screw of lateral neck of vertebra and tension steel-wire to carry out internal fixation.The materials for two groups were recorded 2months,4months and 6months after using the dynamic internal fixation.The collagen typeⅡ of the cartilage endplate was detected with the immunohistochemistry method(each time point 6rabbits),using JD801 picture analytical system for analysis.Then,the degenerative course and mechanism of the collagen typeⅡ cartilage endplate were judged synthetically.Results Compared with that of control group,the tension wire dynamic fixation stability reconstruction for intervertebral instability in experimental group showed significant thichening of the intervertebral cartilage endplate in 2months to 6 months after operation,the cartilage cells correspondingly increased.The gray scale of collagen typeⅡ in experimental group gradually declined from 2months to 6months after operation,the gray scale of collagen typeⅡ in experimental group were all less than those in control group in 2months,4months and 6months after operation(P〈0.01);the optical density of collagen typeⅡ in experimental group gradually increased from 2 months to 6 months after operation,the values were all higher than those of control group(P〈0.01).Conclusion The dynamic internal fixation can effectively prevent the degeneration of t
