摘要
目的观察2型糖尿病下肢血管病变患者介入前后血清高敏C反应蛋白(high-sensitivity C-reactive protein,hsCRP)的变化。方法选择收治的61例下肢血管病变患者为观察组,另选同期健康体检者61例为对照组。记录观察组介入前后的症状表现,检测2组总胆固醇(total cholesterol,TC)、三酰甘油(triglyceride,TG)、低密度脂蛋白胆固醇(Low-density lipoprotein,LDL-C)、高密度脂蛋白胆固醇(high density lipoprotein cholesterol,HDL-C)、糖化血红蛋白,检测观察组不同时点的hsCRP水平。结果观察组介入后较介入前症状明显改善,分别为疼痛24.59%(15/61)vs 68.85%(42/61)、皮温下降26.23%(16/61)vs 59.02%(36/61)、麻木9.84%(6/61)vs70.49%(43/61)、发绀11.48%(7/61)vs 52.46%(32/61),差异均有统计学意义(P<0.01)。观察组介入前TC、TG、LDL-C、糖化血红蛋白明显高于对照组(P<0.01),HDL-C明显低于对照组(P<0.05);介入后观察组TC、TG、LDL-C、糖化血红蛋白均较介入前明显降低,HDL-C较介入前明显升高(P<0.05),介入后观察组各项指标与对照组差异均无统计学意义(P>0.05)。观察组介入后24、48hhsCRP水平较介入前明显升高(P<0.05);介入后1周,hsCRP较介入后24、48h明显降低(P<0.05),但高于介入后2周(P<0.05),与介入前比较差异无统计学意义(P>0.05)。结论血清hsCRP可能为糖尿病启动因子之一;介入后24、48h的血清hsCRP水平显著高于介入前,表明介入后早期的炎症反应明显增强;而介入后2周的血清hsCRP水平较介入后24、48h明显降低,提示介入后炎症反应下降,血流通畅,介入治疗效果显著。
Objective To investigate the change of serum high-sensitivity C-reactive protein in type 2 diabetes mellitus patients with lower extremity vascular disease before and after intervention treatment.Methods The study selected 61 type 2diabetes mellitus patients with lower limb lesions as observation group and 61 cases of health people as control group.The symptoms were recorded before and after the intervention,total cholesterol(TC),triglycerides(TG),low density lipoprotein cholesterol(LDL-C),high density lipoprotein cholesterol(HDL-C),glycosylated hemoglobin,and hsCRP were determined in both groups.Results The symptoms of observation group improved significantly after intervention as compared with before intervention,respectively,pain alleviated 24.59%(15/61)vs 68.85%(42/61),skin temperature dropped26.23%(16/61)vs 59.02%(36/61),numbness 9.84%(6/61)vs 70.49%(15/61),cyanosis11.48%(7/61)vs 52.46%(32/61)(all P〈0.01).Before intervention,TC,TG,LDL-C,glycosylated hemoglobin of observation group were significantly higher than those of control group(P〈0.01),while HDL-C was significantly lower than that of control group(P〈0.01).After intervention,TC,TG,LDL-C,glycosylated hemoglobin of observation group were significantly lower than before intervention,HDL-C significantly increased(P〈0.05).After intervention,observation group showed no significant difference in the indicators with control group(P〈0.05).After intervention for 24 h,48h,observation group showed higher hsCRP level as compared with that after intervention 24,48h(P〈0.05).Then,2 weeks after intervention,hsCRP significantly reduced further more as compared with before intervention(P〈0.05),when hsCRP level showed no statistical significance in comparison with before intervention(P〈0.05).Conclusion Serum hsCRP may start a factor for diabetes mellitus.In 24 hand 48 hafter intervention,serum hsCRP level was significantly higher than before intervention,saying early inflammator
