摘要 目的:探讨螺旋状钢板微创治疗肱骨干骨折的疗效。方法:选取2007-2012年笔者所在医院收治的34例行螺旋状钢板微创治疗的肱骨干骨折患者。所有患者均为闭合性肱骨干骨折,其中包括简单横断及复杂粉碎性骨折。分别以肱骨近端外侧及肱骨远端前侧的两个小切口为入路。将术前备好的波状外形钢板置于肌层以下,其中钢板近端置于外侧而远端置于前侧。螺钉通过间断小切口拧入。所有患者均获得平均6个月随访。肩肘关节的功能康复按照Constant-Mayo肘关节评分标准进行评估。结果:所有切口均一期愈合,无并发症,且所有骨折均获骨性愈合。所有患者肩肘功能均获得优良评分。结论:螺旋状钢板微创治疗肱骨干骨折是一种安全、有效的外科技术。 Objective: To explore the effect of minimally invasive helical plating for humerus shaft fractures.Method: 34 patients with humerus shaft fractures from 2007 to 2012 in our hospital were selected, they treated with minimally invasive helical plating.All the cases had closed humeral shaft fractures, either simple transverse or with mild eomminution.Two small incisions were made.The proximal incision was placed along the deltopectoral groove over the shaft, and the distal incision was placed as in an antero-lateral approach.The pre-selected plate was contoured and introduced in the submuseular plane.The plate was placed in a proximal-lateral and distal-anterior position.On the other hand, the screws were inserted through stab incisions.All the cases were followed for an average of 6 months.Functional recovery of the shoulder and elbow was assessed using the Constant and Mayo elbow performance score systems.Result: All incisions healed by first intention without complications and all the fractures went on to unite.Besides, all cases achieved good to excellent shoulder and elbow function. Conclusion: It is an safe and effective technique to minimally invasive treatment of humerus shaft fractures with helical plate.
