摘要
目的应用多排螺旋CT探讨1型糖尿病患者腹部脂肪含量分布情况,并分析其与临床指标的相关性。方法回顾性分析79例1型糖尿病患者临床资料,对所有患者均进行多排螺旋CT检查并行腹部脂肪含量测定。将1型糖尿病患者按不同年龄和性别进行分组,应用t检验和方差分析比较各组中腹腔内脂肪含量(VA)、腹壁下脂肪含量(SA)、VA/SA的差异性。应用Pearson相关分析分析1型糖尿病患者VA、SA、VA/SA与体质量指数(BMI)、甘油三脂(TG)、高密度脂蛋白(HDL-C)、低密度脂蛋白(LDL-C)以及年龄的相关性。结果 1各年龄组中女性SA、VA大于男性,VA/SA小于男性,而仅有10~20岁、21~30岁组中男、女性SA、VA/SA的差异有统计学意义(P均〈0.05)。男性VA/SA随年龄增长逐渐增长,差异有统计学意义(F=4.67,P〈0.05)。2SA与BMI呈正相关(r=0.54,P〈0.01);VA与BMI、TG、LDL-C呈正相关(r=0.56、0.41、0.21,P均〈0.05);VA/SA与年龄成正相关(r=0.28,P〈0.05)。结论应用多排螺旋CT测量腹部脂肪含量简单可行;1型糖尿病患者腹部脂肪分布在不同年龄、性别组中存在差异;糖尿病患者腹部脂肪含量与临床指标也存在一定的相关性。
Objective To explore the abdominal fat distribution characteristics and its correlation with clinical parameters in type 1diabetes mellitus patients.Methods Totally 79 patients with type 1diabetes mellitus were retrospectively analysed.All patients underwent multidetector spiral CT examination and the abdominal fat distribution were observed and measured.All the patients were grouped according to their age and sex.The difference of visceral adiposity(VA),superficial adiposity(SA),VA/SA were compared by ANOVAand t test.Besides,the correlations between VA,SA,VA/SA and body mass index(BMI),triglycerides(TG),high-density lipoprotein cholesterol(HDL-C),low-density lipoprotein cholesterol(LDL-C),age were analyzed by Pearson correlation.Results 1VA and SA of female patients were higher than that of male patients,while VA/SA of female patients was lower,and the difference of SA,VA/SA in 10—20years old group and 20—30years old group were significant(all P〈0.05).VA/SA of male patients increased with age,and the difference were significant(F=4.67,P〈0.05).2SA was positively correlated with BMI(r=0.54,P〈0.01).VA was positively correlated with BMI,TG,LDL-C(r=0.56,0.41,0.21,all P〈0.05).VA/SA was positively correlated with age(r=0.28,P〈0.05).Conclusion Multidetector spiral CT is a simple and reliable tool in assessing abdominal fat tissue.Besides,there are different abdominal fat distribution among different age and sex patients with type 1diabetes mellitus,and there are certain correlations between abdominal fat distribution and clinical parameters.
