摘要
目的 研究早发型重度子痫前期患者左心室收缩功能变化特点,探索超声技术在重度子痫前期患者血流动力学监护中的应用。方法 选择早发型重度子痫前期患者29例,晚发型重度子痫前期组患者30例,正常2038周妊娠孕妇50例,应用超声心动图技术获得左室收缩功能相关参数：左室射血分数（LVEF）、左室短轴缩短率（FS）,比较各组间左室收缩功能参数的差异。结果 早发型重度子痫前期患者其左心室收缩功能指标LVEF[（59.9±7.22）%]、FS[（32.3±4.93）%]明显低于对照组[LVEF（67.8±5.39）%、FS（37.8±4.46）%]及晚发型重度子痫前期患者[LVEF（68.1±5.92）%、FS（38.2±4.69）%],差异具有统计学意义（P〈0.01）;而晚发型重度子痫前期患者组的LVEF、FS与正常对照组间差异无统计学意义（P〉0.05）。结论 早发型重度子痫前期患者不同于晚发型重度子痫前期患者,存在明显的左室收缩功能减低;超声心动图在早发型重度子痫前期患者期待治疗的血流动力学监护中具有重要意义。
Objective To assess maternal left ventricular systolic function evaluation using echocardiography in early-onset severe preeclampsia women, and explore the hemodynamic monitoring in patients with severe preeclampsia by ultrasonography and echocardiography. Methods Twenty-nine women with early-onset severe preeclampsia were re- cruited by ultrasound screening, 30 women with late-onset severe preeclampsia, and 50 normotensive controls with 20-38 weeks gestation were recruited.The following systolic parameters on echocardiography were measured：left ventricular ejection fraction（LVEF）, left ventricular fractional shortening（FS）. The parameters were studied by comparing the differ- ences. Results Compared with the control group（LVEF 67.8±5.39; FS 37.8±4.46） and late-onset severe preeclampsia group （LVEF 68.1 ±5.92;FS 38.2±4.69）, the indexes during systolic period of early-onset severe preeclampsia decreased significantly the difference of LVEF, FS （59.9± 7.22;32.3 ±4.93） of the two groups were statistically significant （P 〈 0.01）; whereas compared with the control group, and late-onset severe preeclampsia group, there was no statistical signifi- cant difference in the indexes during systolic period （P 〉 0.05）. Conclusion Early onset severe preeelampsia cases exhib- it significant left ventrieular chamber dysfunction compared to the late-onset severe preeclampsia cases. Expectant management combined with hemodynamic monitoring by echocardiography can maximize the safety of pregnant women.
