摘要
目的探究老年髋骨骨折合并脑卒中后遗症患者的临床治疗效果。方法对本院2012年9月至2014年6月收治的138例老年髋骨骨折合并脑卒中后遗症患者的骨折类型进行分型,根据实际情况选择适合患者的治疗方法,治疗后根据患者骨折类型、治疗方案等情况制订康复训练计划,并整理分析患者的治疗方法和效果。结果人工关节置换组患者术后并发症发生率为34.78%(16/46),髓内钉固定组为32.61%(16/46),保守治疗组为58.70%(27/46),人工关节置换组和髓内钉固定组患者术后并发症发生率与保守治疗组比较差异均具有显著性(P<0.05)。人工关节置换组患者治疗有效率为82.61%(38/46),髓内钉固定组为84.78%(39/46),均高于保守治疗组[71.74%(33/46)],差异具有显著性(P<0.05)。结论老年髋骨骨折合并脑卒中后遗症患者具有较多的高危因素,治疗时应根据患者的骨折类型制订治疗方案,治疗后结合患者实际情况制订康复训练计划,提高患者生活质量。
Objective To explore the clinical therapeutic effect of hip fracture complicated by stroke sequela in elderly patients. Method 138 cases of elderly patients with hip fractures complicated by stroke sequelae were selected and typed from September 2012 to June 2014, related treatment methods were chose according to the actual situation of the patient. After treatment, the rehabilitation plan was specified according to the fracture type and treatment programs of patients, the efficacy and treatment methods of patients were analyzed. Result The postoperative complication rate of artificial joint replacement group was 34.78%（16/46）, intramedullary nailing group was 32.61%（16/46）, conservative treatment group was 58.70%（27/46）, the rate of artificial joint replacement group and intramedullary nail fixation group were lower than conservative treatment group, and the difference were significant（P〈0.05）. The effective rate of artificial joint replacement patients was 82.61%（38/46）, intramedullary nail fixation group was 84.78%（39/46）, and they were significantly higher than conservative treatment group [71.74%（33/46）]（P〈0.05）. Conclusion Elderly patients with hip fracture complicated by stroke sequelae had more risk factors, the treatment plan should be developed depending on the type of fracture of patients, and the rehabilitation program after treatment should be combined with the actual situation of patient in order to improve their quality of life.
