球囊成形术治疗胫骨平台塌陷骨折的效果 被引量:2

摘要 目的 探讨球囊成形术治疗胫骨平台塌陷骨折的效果。方法 选择2013年1月~2014年5月乐清市人民医院收治的30例胫骨平台塌陷骨折患者为研究对象,采用随机数字表法将患者分为两组,分别进行球囊成形术(观察组)和传统开放内固定术(对照组)。比较两组临床疗效、术中术后情况、膝关节HSS评分、关节面塌陷程度及并发症发生情况。结果 观察组总有效率为93.33%,对照组患者的总有效率为73.33%,两组比较差异有统计学意义(P〈0.05)。观察组患者的手术时间[(72.14±10.86)min]、术中出血量[(32.56±10.31)m L]、切口长度[(5.87±1.41)cm]、骨折愈合时间[(3.12±0.61)个月]、住院时间[(15.98±3.47)d]均明显低于对照组[(104.57±11.39)min、(100.45±21.09)m L、(14.05±2.76)cm、(6.35±4.53)个月、(23.36±2.89)d],差异均有统计学意义(P〈0.05或P〈0.01)。观察组患者的HSS评分[(82.34±15.67)分]明显高于对照组[(51.89±11.24)分],关节面塌陷程度[(4.08±1.49)mm]低于对照组[(8.96±2.23)mm],差异均有统计学意义(P〈0.05)。观察组患者的并发症总发生率为13.33%,对照组患者的并发症总发生率为33.33%,两组比较差异有统计学意义(P〈0.05)。结论 球囊成形术用于胫骨平台塌陷骨折疗效显著,能够迅速达到解剖复位,降低并发症发生率,安全性高。 Objective To investigate the effect of balloon angioplasty in the treatment of tibial plateau fracture collapse. Methods From January 2013 to May 2014 in the People's Hospital Yueqing City, 30 cases of tibial plateau fractures were collapsed for the study were randomly divided into balloon angioplasty (observation group) and traditional open in- ternal fixation (control group) according to the random number table. The clinical efficacy, intraoperative and postoper- ative cases, HSS scores, subsidence of the articular surface and occurrence of complications in two groups were com- pared. Results The total effective rate of the observation group was 93.33%, and the control group was 73.33%, the difference of two groups was statistically significant (P 〈 0.05). The operative time [(72.14±10.86) min], blood loss [(32.56± 10.31) mL], incision length [(5.87±1.41) cm], fracture healing time [(3.12±0.61) months], hospitalization time [(15.98±3.47)d] of the observation group were significantly lower than those of the control group [(104.57±11.39) rain, (100.45±21.09)mL, (14.05y.2.76) crn, (6.35±4.53) months, (23.36:E2.89) d], the differences were statistically significant (P 〈 0.05 or P 〈 0.01); HSS scores of patients in the observation group [(82.34±15.67) points] was significantly higher than that of the control group [(51.89±11.24) points], the collapse degree of articular surface in the observation group [(4.08±1.49) mm] was lower than that of the control group [(8.96±2.23) ram], the differences of the two groups were statistically significant (P 〈 0.05). The overall incidence of complication in the observation group was 13.33%, and the control group was 33.33%, the difference of the two groups was statistically significant (P 〈 0.05).
