摘要
目的观察健脾安肾方联合针刺治疗肥胖相关性肾病的临床疗效。方法将68例肥胖相关性肾病患者随机分为治疗组和对照组各34例,对照组给予生活方式干预、适当运动、饮食控制,并口服厄贝沙坦片150 mg,每日1次;治疗组在对照组基础上加用健脾安肾方口服,每日1剂,及针刺治疗每次30 min,每日1次。两组患者均治疗9周后评价临床疗效,并于治疗前后检测24 h尿蛋白定量、甘油三酯(TG)、总胆固醇(TC)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)水平及血液流变学指标(包括全血高切黏度、全血低切黏度、血浆高黏度、纤维蛋白原及红细胞压积)。结果两组各32例完成观察。治疗组临床疗效总有效率为90.63%,对照组为62.50%,治疗组临床疗效优于对照组(P<0.05)。两组治疗后24 h尿蛋白定量、TG、TC、LDL-C及血液流变学各指标均明显降低,HDL-C升高(P<0.05),且治疗组各指标改善程度均优于对照组(P<0.05)。结论健脾安肾方联合针刺治疗可降低肥胖相关性肾病患者尿白蛋白量及血脂水平,并纠正血液高凝状态,疗效肯定。
Objective To observe the clinical effects of spleen-invigorating and kidney-soothing prescription combined with acupuncture on 34 cases of obesity-related glomerulopathy. Methods Totally 68 cases obesity-related glomerulopathy were divided into treatment group and control group randomly,with 34 cases in each. Both groups were given lifestyle intervention,appropriate exercise,diet control,and 150 mg of Irbesartan Tablets once daily;Treatment group was added with spleen-invigorating and kidney-soothing prescription orally one dose daily,and acupuncture treatment 30 min each time,once each day. Clinical effects on patients in both groups were evaluated after treatment for 9 weeks,and 24-hour proteinuria,levels of triglyceride（ TG）,total cholesterol（ TC）,high-density lipoprotein cholesterol（ HDL-C）,low density lipoprotein cholesterol（ LDL-C）,hemodynamic indexes（ including whole blood high sheathed viscosity,whole blood low sheathed viscosity,plasma high viscosity,fibrinogen,and hematokrit） were examined before and after treatment. Results Two cases dropped off in each group during the trial,so 32 cases in each group were observed completely. Total clinical effective rate was 90. 63% in treatment group and 62. 50% in control group,with significant difference between the two groups（ P〈0. 05）. Levels of 24-hour proteinuria,TG、TC、LDL-C and every index of hemodynamic all remarkably decreased but HDL-C increased in both group after treatment（ P〈0. 05）,especially in treatment group（ P〈0. 05）. Conclusion Spleen-invigorating and kidney-soothing prescription combined with acupuncture treatment can decrease urinary albumin excretion and blood lipid level and regulate the high blood coagulating state in patients suffering from obesity-related glomerulopathy with a definite curative effect.
