摘要
目的:评估单椎体病变的MR表现,提高MR诊断准确性。方法:回顾性分析47例手术及病理证实的单椎体病变的MR表现,分别测量病灶的对比度及强化程度,评价T1W、T2W、T2W脂肪抑制、T1W增强及T1W脂肪抑制增强检查对病变的显示情况,分析病变的MR表现。结果:T2W脂肪抑制及T1W脂肪抑制增强扫描显示病变的对比度较好。其中29例术前诊断正确(61.7%),12例术后分析修正了影像学诊断,6例因影像学表现缺乏特异性而无法做出修正诊断。26例转移瘤中5例术前误诊,21例其他单椎体病变中13例术前误诊。肿瘤性病变不侵犯相邻椎间盘、血管瘤明显强化及黏液的特殊MR表现对于病变的定性有较大帮助。结论:准确把握不同组织的MR表现有助于提高单椎体病变的MR诊断准确性。
Purpose: To evaluate the MR manifestations of single vertebral lesions and to improve the MR diagnosis accuracy of them. Methods: The MR manifestations of 47 cases pathology confirmed single vertebral lesions were evaluated retrospectively. The contrast degree and enhancement degree were measured respectively. The lesions were evaluated in T1WI, T2WI, T2WI/STIR, post gadolinium enhanced TIWI and contrast enhanced SPIR T1WI. Results: T2WI with fat suppress and contrast enhanced SPIR TIWI were superior in the contrast degree of lesions. Of 47 cases, 29 cases were made correct diagnosis before operation. Of 26 cases metastasis, 5 had wrong diagnosis. Of 21 other cases, 13 cases had wrong diagnosis before operation. After operation the imaging diagnosis of 12 cases was corrected and 6 cases were not diagnosed by the imaging manifestations. The tumors of this group did not involve the adjacent intervertebral disc. The hemangiomas showed with high enhancement. Lesions with mucous showed with special MR manifestations. Conclusion: Understanding the MR manifestations of different tissues may be helpful to improve the diagnosis accuracy of the single vertebral lesions.
