基于Leksell框架的立体定向脑电图(SEEG)技术的临床应用 被引量:5

摘要 目的介绍基于Leksell框架的立体定向脑电图(L-SEEG,Leksell-stereoelectroencephalograph,)系统的工作原理和实现方法,观察其临床安全性,可靠性和易用性。方法 L-SEEG的工作原理和实现方法包括SEEG手术计划软件,Leksell头框以及特殊的配套手术器械三部分,分别予以详细介绍。2014年4月至2015年3月接受L-SEEG技术埋藏电极的顽固性癫痫患者34例,共置入深部电极302根。总结术后颅内出血、脑脊液漏、感染、电极脱落等并发症的情况。结果 L-SEEG稳定可靠,可准确、安全、快速置入深部电极到目标位置。34例患者术后反应轻微,仅1例脑室内出血3ml(无症状),无脑脊液漏、无感染,电极固定可靠。结论 L-SEEG技术是一种安全、可靠、准确,操作方便、相对廉价的SEEG技术,可以在癫痫外科推广应用。 Objective To introduce a new stereotactic EEG(SEEG)Technique based on Leksell stereotactic frame(L-SEEG)and to observe the safety,accuracy,reliability and usability of L-SEEG technique.Methods L-SEEG technique include SEEG operation plan software(Neurotech),Leksell stereotactic frame and a few special surgical instruments.We will introduce them in detail.From Apr 2014 to Mar 2015 we have implanted 302 intracranial electrodes in 34 refractory epilepsy patients through L-SEEG technique.We observed the postoperative complications such as bleeding,cerebral spinal fluid(CSF)leakage,infection,and electrode problems(pulling out and fragment left).Results All electrodes were implanted to the accurate position as we planned before operation.There was no sever complication after intracranial electrodes implantation through L-SEEG technique.There were no electrode problems(pulling out and fragment left),no cerebral spinal fluid(CSF)leakage and no infection during intracranial EEG monitoring.Only 1patient had frontal lateral ventricle hematoma(3ml)without any clinical symptom.Conclusion L-SEEG is a safe,reliable,precise and relatively cheap SSEG technique to implant intracranial electrodes.It can be applied universally in epilepsy surgery.
