加味大黄承气汤联合生长抑素治疗重症急性胰腺炎疗效观察及对血清钙和细胞因子水平的影响 被引量:16

摘要 目的:探讨加味大黄承气汤联合生长抑素治疗重症急性胰腺炎疗效及对血清钙和细胞因子水平的影响。方法:62例患者根据随机数字表法随机分为观察组与对照组各31例。两组患者均给予常规对症治疗。对照组在此基础上给予生长抑素治疗,观察组在对照组基础上服用加味大黄承气汤。两组疗效均为14 d。结果:观察组总有效率(93.55%)显著高于对照组(70.97%),且具有统计学差异(P<0.05);治疗后观察组血清钙水平显著高于对照组(P<0.05);观察组治疗后排气恢复时间、腹痛消失时间及平均住院时间均显著短于对照组(P<0.05);治疗后观察组IL-6、TNF-α含量明显低于对照组,IL-10含量明显高于对照组(P<0.05)。结论:加味大黄承气汤联合生长抑素治疗重症急性胰腺炎疗效显著,可明显提高患者血清钙水平,降低细胞因子水平,从而减轻患者炎症反应,具有临床意义。 Objective: To investigate combined effect of Modified Dachengqi Tang and somatostatin in the treatment of severe acute pancreatitis and the effects on serum calcium and cytokine levels. Methods : 62 patients were randomly divid- ed into two groups ( n = 31 ) and control group ( n = 31 ) based on the random number table. All patients were treated with conventional symptomatic treatment. The control group received somatostatin therapy and based on this observation group took Modified Dachengqi Tang. Two groups were treated 14 d. Results: The overall response rate (93.55%) was significantly higher (70.97%) (P 〈 0.05 ) ;in observation group after treatment, serum calcium levels were significantly higher (P 〈 0.05 ) ; exhaust recovery time, pain disappeared and average length of stay were significantly shorter than those of the control group (P 〈 0.05 ) ; after treatment, observation group's IL - 6 and TNF - α were significantly lower than those of the control group and IL - 10 was significantly higher( P 〈 0.05 ). Conclusion: Modified Dachengqi Tang and somatostatin treatment of severe acute pancreatitis has significant effect and can significantly increase serum calcium levels in patients with lower levels of cytokines, and thus reducing inflammation in patients with clinical significance.
