摘要
目的观察分析颈部冰敷联合氧气射流雾化防治气管内麻醉后肺部感染的临床效果,减少肺部感染发生。方法选取2012年3月-2013年3月收治的89例行气管切开术患者,随机分为观察组45例和对照组44例,观察组患者采用颈部冰敷联合氧气射流雾化方法,对照组采用雾化吸入治疗,对比两组患者气道湿化效果。结果两组患者气道湿化痰栓形成、刺激性咳嗽对比,差异无统计学意义;观察组有8.9%的患者出现气管黏膜出血,明显低于对照组的13.6%,差异有统计学意义(P<0.05);观察组患者肺部感染率为46.7%、对照组为77.3%,两组对比差异有统计学意义(P<0.05);痰液Ⅲ度黏稠度观察组患者为4.5%,明显要低于对照组的34.1%,差异有统计学意义(P<0.05)。结论颈部冰敷联合氧气射流雾化能够有效防治气管内麻醉后肺部感染、减少患者的疼痛、降低痰液的黏稠度。
OBJECTIVE To observe the clinical effect of ice compress on neck combined with oxygen jet nebulization in the prevention of pulmonary infections after endotracheal anesthesia so as to induce the pulmonary infections . METHODS From Mar .2012 to Mar 2013 ,totally 89 cases of patients received tracheotomy were selected and they were randomly divided into observation group with 45 cases and control group with 44 cases .The observation group was treated with ice neck combined with oxygen jet atomization and the control group was with inhalation therapy .The clinical effects were compared .RESULTS The comparison of sputum and irritable cough in the two groups of patients with airway humidification was without significance .Totally 8 .9% of patients appeared hemor‐rhage on tracheal mucosa in the observation group ,which was significantly lower than the control group with 13 .6% (P〈0 .05) .The pulmonary infection rate in the observation group was 46 .7% and in the control group ,it was 77 .3% .The differences were significant (P〈0 .05) .A total of 4 .5% of patients in the observation group had sputum with viscosity of degree III ,which was significantly lower than the control group of 34 .1% (P〈0 .05) . CONCLUSION The ice compress on neck combined with oxygen jet nebulization has good effect on the prevention of pulmonary infections after endotracheal anesthesia ,pain relief and thick sputum .
