深度水解蛋白配方奶在早期早产儿喂养中的疗效观察 被引量:16

摘要 目的评估深度水解蛋白配方奶(e HPF)与标准早产儿配方奶(SPF)在早产儿喂养中对喂养不耐受、营养摄入、生长及生化指标的影响,探讨其使用价值。方法回顾性分析2013年1月—2014年8月该科新生儿重症监护病房(NICU)收治的早产儿(胎龄≤32周或出生体重<1 500 g)中深度水解蛋白配方奶的喂养情况,将120例随机分为e HPF组(60例)和SPF组(60例),观察及比较两组患儿头围、体重、身长,每天牛奶摄入量,喂养不耐受情况、达全胃肠喂养时间及相应生化指标,住院天数,宫外发育迟缓(EUGR)发生率,并分析两组间有无统计学差异。结果与SPF组相比,e HPF组喂养不耐受发生率减低(0.425 vs 0.667),达全胃肠营养时间缩短[(10.9±2.6)d vs(12.6±3.0 d)],三周血清总蛋白[(47.5±2.6)、(46.4±2.3)、(48.6±0.9)g·L-1]及白蛋白[(26.8±2.1)、(27.1±2.5)、(28.1±1.9)g·L-1]增高,差异有统计学意义(P<0.05);两组之间体重增长在第一周及第二周差异无统计学意义,而在第三周e HPF组与SPF组相比体重增长较快[(1 826.4±115.7)g vs(1 773.8±108.9 g)],差异有统计学意义;头围、身长增长及尿素氮水平差异无统计学意义(P>0.05)。e HPF组患儿总胆红素水平、直接胆红素水平、住院时间、胆汁淤积的发生率及坏死性小肠结肠炎发生率均低于SPF组(P<0.05)。结论深度水解蛋白配方奶能减少早产儿喂养不耐受的发生率,缩短全胃肠喂养时间,提高血清蛋白水平,降低感染性疾病的发生,缩短住院时间,值得临床推广使用。 Objective To compare feeding tolerance,nutrient intake,growth and serum chemistries in preterm infants fed either a standard preterm infant fomula(SPF)or an extensively hydrolyzed protein formula (eHPF),and to explore the clinical application val-ue.Methods One hundred and twenty preterm infants(gestational age ≤32 w or birth weight 〈1 500 g)who were admitted to NICU of Anhui Provincial Hospital from Jan.2013 to Aug.2014 were randomized into eHPF group and SPF group.Both groups were fed either formula at least 3 weeks.Intake was monitored daily.Measurement of body weight every other day,weekly monitoring of biochemical markers,head circumference and height,feeding intolerance,starting time of full enteral feeding,hospitalization days and EUGR inci-dence were compared.Results Compared with the SPF,eHPF group had reduced incidence of feeding intolerance (0.425 vs 0.667), shortened total parenteral nutrition [(10.9 ±2.6)d vs (12.6 ±3.0 d)],increased three -week serum total protein [(47.5 ±2.6), (46.4 ±2.3),(48.6 ±0.9)g·L^ -1 ] and albumin [(26.8 ±2.1),(27.1 ±2.5),(28.1 ±1.9)g·L ^-1 ],which had significant differences(P 〈0.05).There were no significant differences in weight gain in the first week and the second week between the two groups,but in the third week weight gain in eHPF group grew rapidly [(1826.4 ±115.7)g vs (1773.8 ±108.9 g)],which had statis-tically significant difference.Head circumference,height growth and urea nitrogen levels showed no significant difference (P 〉0.05). Total bilirubin,direct bilirubin,hospitalization days,incidence of cholestasis and incidence of enterocolitis in eHPF group were lower than those in SPF group (P 〈0.05).Conclusions eHPF can reduce the incidence of feeding intolerance,shorten full enteral feeding time and hospitalization,decrease the incidence of NEC and increase serum protein levels.
