摘要
目的观察扩大切除额极胶质瘤的临床疗效。方法 2010年6月至2013年6月我院接诊的100例额极胶质瘤患者,其中50例行扩大切除肿瘤手术治疗(观察组),另50例采用不扩大切除手术治疗(对照组)。比较两组患者术后KPS评分、中位生存期、复发人数、头痛和癫痫治疗效果与并发症。结果观察组术后头痛、癫痫治疗效果、KPS评分及中位生存期均优于对照组,复发率明显低于对照组,差异均有有统计学意义(P<0.05)。两组术后并发症发生率比较,差异无统计学意义(P>0.05)。观察组低病理类型患者的术后KPS评分、中位生存期均高于高病理类型患者,复发人数少于高病理类型患者,差异有统计学意义(P<0.05)。结论扩大切除额极胶质瘤疗效良好,能显著提高治疗效果和降低复发率,值得临床推广。
Objective To observe the efficacy of extended resection of gliomas in frontal pole.Methods One hundred patients with gliomas in frontal pole from June 2010 to June 2013 in our hospital were included in the study.They were randomly divided into observation and control groups,50 in each group.The observation group was treated through extended resection while the control group was treated with non-expanded resection surgery.Clinical data,postoperative KPS,median survival period,number of relapse,treatment effects of headaches and epilepsy and complications were compared between the two groups.Results After treatment,the effect of headache,the epilepsy disappearance,KPS and the median survival time in the observation group were better than that in the control group.Postoperative KPS score in the observation group was higher than that in the control group（P〈0.05）.The median survival time in the observation group was longer than that in the control group,and the relapse rate in the observation group was lower than that in the control group（all P〈0.05）.No statistical significance in postoperative complications ratio was found between the two groups.Postoperative KPS,the median survival time of patients with low histological type was higher than that of patients with the high histological type.The number of relapse in patients with low histological type was less than that in patients with the high histological type（P〈0.05）.Conclusion Efficacy of extended resection for gliomas in frontal pole was good.It can significantly improve the therapeutic effect and reduce the recurrence rate.It is worthy of promotion in clinic.
