摘要
目的:观察云南白药联合珍珠粉保守治疗精神分裂症并发应激性溃疡出血(SUB)临床疗效与安全性。方法:73例精神分裂症并发SUB,经内镜下止血后Forrest镜下分级达到Ⅱc、Ⅲ的低危象征患者随机分为对照组(37例)和观察组(36例)。两组皆给予胃管喂饲给药,抗精神病药物为阿立哌唑片,对照组予奥美拉唑胶囊,观察组予云南白药胶囊加珍珠粉胶囊治疗。以实验室指标、阳性与阴性症状量表(PANSS)和不良反应量表(TESS)为评价指标,评价两组患者的疗效和安全性。结果:两组止血效果总有效率均为100%(P>0.05)。内镜下观察,2种方法止血效果明显,差异无统计学意义(P>0.05)。治疗2周末、4周末,观察组PANSS减分率高于对照组(P<0.05),TESS评分低于对照组(P<0.05)。而两组不良反应发生例数比较,差异无统计学意义(P>0.05)。结论:在保守治疗精神分裂症并发SUB时,云南白药联合珍珠粉的疗效与奥美拉唑相当,但安全性略高于奥美拉唑,在临床有推广应用价值。
Objective： To observe the clinical efficacy and safety of Yunnan Baiyao combined pearl powder in the conservative treatment of schizophrenia complicated with stress ulcer bleeding （SUB）. Methods ： Totally 73 cases of schizophrenia patients with SUB after endoscopic hemostasis and Forrest microscopically graded as Ⅱ c or Ⅲ low-risk symbol symbo were randomly divided into the control group （37 cases） and the observation group （36 cases）. The two groups were both given aripiprazole tablets through gastric tube, and the control group was given omeprazole capsules additionally while the observation group was treated with Yunnan Baiyao capsules combined with pearl powder capsules. The laboratory indices, positive and negative symptom scale （PANSS） and adverse reaction scale （TESS） as the evaluation indices, the efficacy and safety were evaluated. Results： The total effective rate of hemostasis of the two groups was 100% （P 〉 0.05）. With the endoscopic observation, the hemostatic effect of the two methods was obvious with no statistically significant difference （ P 〉 0.05 ）. After the 2- and 4-week treatment, the reduction rate of PANSS in the observation group was higher than that in the control group （P 〈 0.05）, and TESS in the observation group was lower than that in the control group （P 〈 0.05 ）. The incidence of ADR in the two groups showed no statistically significant difference （ P 〉 0.05 ）. Conclusion： In the treatment of schizophrenia with SUB, the curative effect of Yunnan Baiyao combined with pearl powder is the same with that of omeprazole with higher security, which is valuable to be promoted in clinics.
