摘要
目的 分析图片动词联想、单纯图片命名、词语语义判断3种视觉呈现语言任务在汉语母语者中的血氧水平依赖功能磁共振成像(BOLD-fMRI)语言区定位特点,并探讨其在神经外科临床中的应用. 方法 12名健康志愿者分别执行图片动词联想、单纯图片命名、词语语义判断任务,所得图像数据应用SPM8软件进行分析,生成不同语言任务的脑激活图后进行研究,并计算不同语言任务的偏侧化指数. 结果 3种语言任务在额叶及颞叶均出现了明显的左侧偏侧化,图片动词联想任务激活了包括左额下回(BA44、BA45)、左岛叶(BA13)、左颞中回(BA39)、左颞横回(BA41)、左颞上回(BA41)、左顶下小叶(BA40)、左缘上回(BA40)、左角回(BA39)等传统语言区在内的广泛皮层及皮层下网络;而单纯图片命名激活了部分传统皮层语言区,如左额下回(BA44、BA45)、左颞中回(BA39)、左顶下小叶(BA40)、左颞上回(BA22);词语语义判断任务也激活了部分传统皮层语言区,如左额下回(BA44、BA45). 结论 图片动词联想、单纯图片命名、词语语义判断任务均可激活语言网络中的多个皮层脑区,不同语言任务的激活区体现不同的认知过程及加工特点,图片动词联想更适合汉语母语者神经外科术前BOLD-fMRI语言区定位.
Objective To investigate the activated brain regions of functional MRI with 3 language tasks in normal adult subjects whose native language is Chinese and provide reference to the selection of proper language task to mapping language areas.Methods Twelve right-handed normal volunteers whose native language was Mandarin participated in this study.They underwent functional MRI with a 1.5 T MR scanner when performing picture verb generation (VG),pure picture naming (PN) and abstract/concrete semantic judgment (A/C) tasks,respectively.The SPM8 software was used to analyze the data to obtain functional activated regions and calculate the lateralization indexes.Results The lateralization indexes indicated a significant lateralized activation of left hemisphere by all 3 language tasks.As to the activation of traditional language areas,the VG task activated left inferior frontal gyrus (BA44 and BA45),left insula (BA13),left middle temporal gyrus (BA39),left transverse temporal gyrus (BA41),left superior temporal gyrus (BA41),left inferior parietal lobule (BA40),left supramarginal gyrus (BA40),left angular gyrus (BA39).The PN task activated left inferior frontal gyrus (BA44 and BA45),left middle temporal gyrus (BA39),left inferior parietal lobule (BA40),left superior temporal gyrus (BA22).The A/C task only activated left inferior frontal gyrus (BA44 and BA45).Conclusions Each of the 3 language tasks can activate some parts of the cerebral network which dedicates to language processing and has its own characteristic according to the different processing pattern.VG task is more appropriate for preoperative language mapping for Chinese subjects.
