摘要
目的探讨吡格列酮联合多烯磷脂酰胆碱治疗非酒精性脂肪肝的临床效果。方法将157例非酒精性脂肪肝患者采用数字表法随机分为研究组79例和对照组78例。对照组给予多烯磷脂酰胆碱治疗,研究组给予吡格列酮联合多烯磷脂酰胆碱治疗,疗程一个月,治疗两个疗程。对两组患者进行相关性生理指标的检测和检查、超声检查、同时比较两组患者的临床疗效和不良反应发生情况。结果两组患者的丙氨酸氨基转移酶、谷氨酰转肽酶、天冬氨酸氨基转移酶、甘油三酯、总胆固醇和体质量指数均较治疗前显著降低(P<0.05),且治疗后研究组的丙氨酸氨基转移酶、甘油三酯和总胆固醇与对照组比较均有统计学差异(P<0.05)。研究组患者超声影像学疗效显示,除纹理管消失外其他不良症状均显著少于对照组(P<0.05)。研究组和对照组患者的治疗有效率分别为88.6%和75.6%,两组间存在统计学差异(P<0.05)。两组患者不良反应发生率分别为1.3%和7.6%,两组间存在统计学差异(P<0.05)。结论使用吡格列酮联合多烯磷脂酰胆碱治疗非酒精性脂肪肝效果更佳,且不良反应更少,安全有效具有较好临床应用价值。
Objective To investigate the effects of pioglitazone combined with polyene phosphatidylcholine in treatment of nonalcoholic fatty liver disease.Methods 157 patients with non alcoholic fatty liver were randomly divided into study group(79cases)and control group(78cases).The control group was given polyene phosphatidylcholine treatment,study group combined with polyene phosphatidylcholine treatment.The physiological index,ultrasonic inspection,at the same time,the clinical efficacy and adverse reactions of two groups were compared.Results The alanine amino transferase,glutamyl transpeptidase,aspartate aminotransferase,triglyceride,total cholesterol and body mass index of the two groups were significantly decreased after treatment(P〈0.05).Alanine aminotransferase,triglyceride and total cholesterol of study group after treatment were different from that of control group(P〈0.05).To show the efficacy of ultrasound imaging in patients of the study group,in addition to texture effacement cases other adverse symptoms were significantly less than that of the control group(P〈0.05).The effective rate of study group and the control were 88.6%and 77.2%(P〈0.05).The adverse reaction rate of study group and the control were 1.3% and 7.6%(P〈0.05).Conclusion The use of pioglitazone combined with polyene phosphatidylcholine in treatment of nonalcoholic fatty liver have better effect,and less adverse reaction.
