The performance of contrast-enhanced ultrasound for focal pancreatic lesions

摘要 目的分析胰腺良、恶性局灶性病变超声造影表现。方法对疑有胰腺病变的35例患者分别行常规超声和超声造影检查,结合手术后病理结果,对比分析其超声造影增强水平、增强速度及增强模式。结果 35例胰腺病变经手术病理证实或临床确诊为恶性病变28例,良性病变7例。其中25例胰腺腺癌全程为整体不均匀低增强,24例晚增强,1例同步增强;3例早退,19例晚退,3例同时消退。2例神经内分泌癌动脉相不均匀缓慢高增强,静脉相低增强;1例晚退,1例早退。1例转移癌动脉相不均匀缓慢低增强,静脉相高增强并晚退。1例胰岛细胞瘤动脉相不均匀快速高增强,静脉相呈等增强并早退。2例假性囊肿始终无增强。2例囊腺瘤动脉相不均匀等增强,静脉相低或等增强;1例早增强并早退,1例晚增强并晚退。2例局灶性胰腺炎与胰腺实质同时增强和消退。结论超声造影可以弥补常规超声不足,通过不同增强模式对胰腺占位行病变进行定性诊断,并且能够对胰腺肿瘤的血流灌注提供更可靠的诊断依据。 Objective To analyse the ultrasonographic characteristics of benign and malignant focal pancreatic lesions as examined by contrast-enhancement ultrasound (CEUS).Methods Thirty-five patients with suspected focal lesions in the pancreas underwent conventional ultrasonography and CEUS examination.The ultrasonographic characteristics were comparatively analyzed in combination with the surgical and pathologic findings.The enhancement level,rate and pattern of CEUS were also analyzed.Results Among 35 cases,28 of them were malignant and the rest 7 cases were benign as confirmed by surgical pathology.The 25 maligant cases showed low and uneven enhancement as examined by CEUS,24 of them showed delayed enhancement and the other one showed syncronized enhancement.The enhancement faded away early in 3 cases,lately in 19 cases and syncronously in 3 cases.The two cases of neuroendocrine carcinoma showed slow and high enhancement during the arterial phase and low enhancement during the venous phase.The enhancement faded away lately in one case and early in other case.One case of metastatic carcinoma showed non-uniform slow and low enhancement during the arterial phase but showed high enhancement during the venous phase,and faded away lately.One case of pancreatic islet cell tumor arterial phase showed uneven,quick and high enhancement during the arterial phases and showed equal enhancement during the venous phase.No enhancement was found in 2 cases of pseudocyst.Two cases of cyst-adenoma inhomogeneous showed equal enhancement during the arterial phase and venous phase is lowly or equally,1 case of enhanced early and left early,1 case of enhanced lately and stayed lately,2 cases of focal pancreatitis with pancreatic parenchyma enhanced with the refund.Conclusion Dynamic perfusion and the characteristics of the microvascular distribution in the lesion and tissue can be reflected by contrast-enhancement ultrasound.It had higher accuracy of displaying the range of the pancreatic cancer and the metastases in the liver and retrop
