摘要
目的探讨核磁共振与超声在胎儿小脑延髓池增宽(Broadening cisterna magna)中的诊断价值。方法经超声检查怀疑胎儿小脑延髓池增宽的孕妇24例,在系统超声检查后48小时内进行胎儿MRI检查。将MRI、超声、随访结果进行对照比较研究。结果 22名孕妇共检查出胎儿24例,MRI证实胎儿小脑延髓池增宽22例,占超声诊断的91.7%(22/24),合并中枢神经其他异常者12例,其中胼胝体发育不良1例,侧脑室增宽4例,Dandy-Walke7例,检出超声漏诊合并侧脑室增宽2例,胼胝体发育不良1例,Dandy-Walker畸形2例。结论核磁共振检查对小脑延髓池增宽的诊断价值明显高于超声,有助于明确诊断胎儿小脑延髓池增宽和合并畸形的诊断。
Objective: To explore the diagnostic value of magnetic resonance image (MRI) in combination with ultrasound (US) in fetal fetal broadening of cisterna magna. Methods: Twenty-four pregnant women were suspected fetal broadening of cisterna magna by by prenatal US.MRI were exeamed in 48 hours after US.The results of MRI and US were compared with following up datas. Rresults: Twenty-four fetuses were detected in 24 pregnant women. Twenty-two fetuses were confirmed broadening of cisterna magna with MRI in 24 cases, twelve fetus were found additional central nervous system (CNS) abnormalities. One fetus were confh'med agenesis of corpus caUosum (ACC) , four fetuses were confirmed expansion of the lateral ventricle, seven fetus was Dandy-Walker abnormality. MRI demonstrated more associated abnormalities than US in 5 fetuses, two was expansion of the lateral ventricle, one was ACC, two was Dandy-Walker abnormality. Conclusion: MRI is superior to US in the diagnosis of fetal expansion of the lateral ventricle, better show the combined brain malformation of fetal broadening of cisterna magna.
