冠心病患者血清黏附分子含量检测及对病变严重程度的判断价值 被引量:13

摘要 目的:研究冠心病患者血清黏附分子含量及其对病变严重程度的判断价值。方法:将2013年5月-2014年8月期间在我院门诊和病房就诊的冠心病患者纳入研究的冠心病组,包括单支病变组、双支病变组、三支病变组患者,各40例,稳定性心绞痛(SAP)组、不稳定型心绞痛(UAP)组、非ST段抬高的急性心肌梗死(NSTEMI)组、ST段抬高的急性心肌梗死(STEMI)组患者,各30例;筛选同期在我院体检的健康者60例纳入研究的健康组。比较各组间的血清黏附分子含量。结果:冠心病组血清中E-选择素、单核细胞黏附分子(intercellular cell adhesion molecule,ICAM-1)、血管细胞黏附分子-1(vascular cell adhesion molecule,VCAM-1)、淋巴细胞功能相关抗原-1(lymphocyte function-associated antigen-1,LFA-1)、CD40含量高于对照组(P〈0.05);三支病变组患者的E-选择素、ICAM-1、VCAM-1、LFA-1、CD40含量高于单支病变组、双支病变组(P〈0.05);UAP患者、NSTEMI患者、STEMI患者的E-选择素、ICAM-1、VCAM-1、LFA-1、CD40含量高于健康者、SAP患者(P〈0.05)。结论:冠心病患者血清中E-选择素、ICAM-1、VCAM-1、LFA-1、CD40的含量异常升高,且黏附分子含量可以判断冠脉病变数目和病变程度。 Objective: To study the serum adhesion molecules contents in patients with coronary heart disease and its prognosis value. Methods: Patients with coronary heart disease admitted from 2013 May to 2014 August were enrolled in ob servation group, including single vessel lesion group, double vessel lesion group and three lesions group, with 40 cases in each group 40 cases; stable angina group, unstable angina group, non ST segment elevated acute myocardial infarction group, ST segment elevation acute myocardial infarction group, with 30 eases in each group. Another 60 eases of health subjects were selected as control group. Serum adhesion molecule levels were detected and compared. Results: Serum E selectin, ICAM 1, VCAM-1, LFA-1, CD40 contents of coronary heart disease group were higher than those of health group; serum E-selectin, ICAM 1, VCAM-1, LFA-1, CD40 contents of three lesions group were higher than those of single vessel lesion group, double vessel lesion group; serum E selectin, ICAM-1, VCAM 1, LFA-1, CD40 contents of NSTEMI group and STEMI group were higher than those of SAPgroup, UAP group. Conclusions: Serum E selectin, ICAM-1, VCAM-1, LFA-1, CD40 contents are abnormally increased in patients with coronary heart disease. Serum adhesion molecules contents can help to judge the number of coronary artery lesions and degree of pathological changes.
