摘要
目的检测耐碳青霉烯类鲍氏不动杆菌中OXA型碳青霉烯酶,探讨鲍氏不动杆菌的耐药机制。方法收集医院2011年1月-2012年12月分离自住院患者痰液标本,经VITEK-2Compact全自动微生物鉴定药敏分析系统鉴定的耐碳青霉烯类鲍氏不动杆菌,采用限制性内切酶分析技术方法进行鉴定,并使用多重PCR方法检测OXA型碳青霉烯酶基因。结果 212株鲍氏不动杆菌中200株鉴定为耐碳青霉烯类鲍氏不动杆菌,其对二、三、四代头孢菌素均耐药,对氟喹诺酮类抗菌药物耐药率达98.37%;200株耐碳青霉烯类鲍氏不动杆菌均表达OXA-51,其中OXA-23-like基因155株;155株OXA-23阳性的耐碳青霉烯类鲍氏不动杆菌,104株分离自ICU住院患者,占67.10%。结论表达OXA-23-like型碳青霉烯酶是耐碳青霉烯类鲍氏不动杆菌的主要耐药机制,鲍氏不动杆菌的耐药形势严峻,尤其是耐碳青霉烯类鲍氏不动杆菌。
OBJECTIVE To detect the OXA‐type carbapenems in carbapenem‐resistant Acinetobacter baumannii and explore the drug resistance mechanisms of the A .baumannii .METHODS The sputum specimens were collected from the patients who were hospitalized from Jan 2011 to Dec 2012 ,then the carbapenem‐resistant A .baumannii strains were identified by using VITEK‐2 Compact automatic microorganism identification and drug susceptibility analysis system , the identification was performed with the use of ARDRA method , and the OXA‐type carbapenemases gene was detected by using multiplex PCR method .RESULTS Of 212 strains of A .baumannii ,200 strains were identified as the carbapenem‐resistant A .baumannii and were resistant to the second ,third ,and fourth generation cephalosporins ,and the drug resistance rate to fluoroquinolones reached up to 98 .37% .All of the 200 strains of carbapenem‐resistant A .baumannii were tested positive for OXA‐51 ,of which 115 strains carried with OXA‐23‐like gene .Of the 155 strains of OXA‐23‐positive carbapenem‐resistant A .baumannii ,104 strains were isolated from the patients hospitalized in the ICU ,accounting for 67 .10% .CONCLUSIONS The expression of OXA‐23‐like carbapenemase is the major drug resistance mechanism of the carbapenem‐resistant A . baumannii ,and the drug resistance of the A .baumannii strains remains harsh ,especially the carbapenem‐resistant A .baumannii .
