摘要
目的探讨RPH联合保留齿线术治疗Ⅲ、Ⅳ期混合痔的有效性和安全性。方法选择92例Ⅲ、Ⅳ期混合痔患者,随机分观察组和对照组,每组46例,观察组采用RPH联合保留齿线术,对照组采用传统外剥内扎术,观察两组疗效、术后并发症(疼痛、出血、肛缘水肿、皮赘形成、排尿障碍等)、创面愈合时间情况。结果观察组有效率为95.7%,对照组有效率为82.6%,两组间比较差异有统计学意义(P<0.05)。观察组术后并发症中的疼痛、出血、排尿障碍、肛缘水肿、皮赘形成的发生率均明显低于对照组,观察组创面愈合时间明显优于对照组,差异有统计学意义(P<0.05)。结论 RPH联合保留齿线术治疗Ⅲ、Ⅳ期混合痔取得良好的疗效,与传统外剥内扎术相比,操作简便,损伤小,术后并发症轻,术后恢复快,值得临床推广应用。
Objective To investigate the efficacy and safety of RPH with preserved dentate line in treatment ofⅢ、Ⅳ mixed hemorrhoids.Methods A total of 92 patients were included in this study,all the patients were randomly divided into treatment and control groups.Treatment group were treated with RPH,control group were treated with traditional external dissection and internal ligation.Efficacy,postoperative complications（pain,blood loos,anal dedma,perianal skin tag and retention of urine）and wound healing time were Observed.Results The total effective rate of treatment group was 95.7%,the control group was82.6%,the differences between the two groups were significant（P〈0.05）;The incidence of pain,blood loos,anal dedma and perianal skin tag in treatment group was significantly lower than the control group（P〈0.05）;the wound healing time of control group were longet than treatment group（P〉0.05）.Conclusion RPH with preserved dentate line is more effective than traditional external dissection and internal ligation in treatment ofⅢ、Ⅳ mixed hemorrhoids,it is simple operation,small injuries,less complications and quicker recovery,deserve the clinical expansion.
