摘要
目的探讨术前放疗联合小切口胸段食管癌根治术治疗胸段食管癌疗效及其对患者肺功能的影响,为临床治疗食管癌提供相应依据。方法收集2005年6月至2008年6月收治的140例胸段食管癌患者作为研究对象。随机分为对照组70例和观察组70例,对照组给予直接小切口食管癌根治手术治疗;观察组给予术前放疗与手术治疗。比较两组食管癌患者并发症、肺功能及1年和5年生存率。结果两组患者吻合口瘘与喉返神经损伤发生率差异无统计学意义;观察组肺部并发症发生率为2.9%,显著低于对照组患者18.6%,差异具有统计学意义(P<0.05)。经过治疗后,观察组肺活量(VC)、1s用力呼气容积(FEV1s)以及最大通气量(MVV)较对照组显著升高,差异均具有统计学意义(P均<0.05)。两组患者1年生存率差异无统计学意义(P>0.05)。观察组患者5年生存率(37.1%)显著高于对照组(20.0%),差异具有统计学意义(P<0.05)。结论术前放疗联合小切口胸段食管癌根治术治疗胸段食管癌可在一定程度上减少手术对患者肺功能的损伤,并提高患者5年生存率,值得深入研究与推广应用。
Objective To investigate the efficacy and effect on the pulmonary function of preoperative radiotherapy combined small inci-sion radical operation for thoracic esophageal cancer( TEC)patients and provide clinical basis for clinical therapy. Methods From June 2005 to June 2008,we collected 140 TEC cases and divided them into 2 groups randomly,70 cases in control group and 70 cases in observation group. The control group( CG)was treated with small incision radical operation and the observation group( OG)was treated with preoperative radiotherapy before surgery. Postoperative complications,the pulmonary function,and the 1 year and 5 year survival rates of the 2 groups were observed and compared. Results The incidence of anastomotic leakage and recurrent laryngeal nerve injury of the 2 groups had no significant difference. The incidence of pulmonary complications of the OG was 2. 9%,which was significantly lower than that of the CG(18. 6%),P 〈0. 05. The level of VC,FEV1s,MVV of the OG were all significantly higher than those of the CG,P 〈0. 05. The 1 year survival rate of the 2 groups had no signifi-cant difference. But the 5 year survival rate of the OG was significantly higher than that of the CG( P 〈0. 05). Conclusion Preoperative radio-therapy combined small incision radical operation for TEC patients was effective and can significantly improve the pulmonary function after treat-ments. Our study provides new evidence for clinical therapy for of TEC.
