摘要
目的探讨不同种类抗菌药物治疗新生儿感染性肺炎的临床疗效及时间效应。方法选取2012年7月至2014年6月医院新生儿感染性肺炎患儿183例,随机分为青霉素类组(A组)、头孢菌素组(B组)和联合用药组(C组),各61例,分别给予注射用美洛西林钠舒巴坦钠、注射用头孢噻肟钠以及上述两种药物联合治疗。另选择61例正常新生儿作为对照组(D组)。观察4组患儿的临床疗效,检测肠道菌群数量,分析时间效应的关系,记录不良反应的发生情况。结果 A组、B组、C组的总有效率分别为95.08%,91.80%和96.72%(P>0.05);平均治愈时间分别为(4.70±1.92)d,(7.94±4.31)d和(4.63±1.66)d,平均治愈时间A组与C组相当(P>0.05),但均短于B组(P<0.05);3组患儿体内肠道菌群状况与对照组比较,有显著性差异(P<0.05),但A组差异最小(P<0.05);A组、B组、C组不良反应发生率分别为11.48%,14.75%和24.59%,A组与B组比较,差异无统计学意义(P>0.05),两组均低于C组(P<0.05)。结论单用青霉素类药物治疗新生儿感染性肺炎可取得良好的临床疗效和时间效应,较低水平的肠道菌群影响,且不良反应少,是治疗的首选药物。
Objective To investigate the clinical effect and time effect of different kinds of antibacterial drug in treating neonatal infectious pneumonia.Methods 183 cases of neonatal pneumonia in ourhospital from July 2012 to June 2014 were selected as the research subjects and randomly divided into the penicillin group,cephalosporins group and combination medication group,61 cases in each group.The three groups were given meziocillin sodium sulbactam sodium for injection,cefotaxime sodium for injection and their combination.Other 61 normal neonates were selected as the control group.The clinical effects were observed in the three treatment groups.The intestinal flora number was detected.The relationship of time effect was analyzed and the occurrence situation of adverse reactions were recorded.Results The total effective rates in the penicillin group,cephalosporins group and penicillins combined cephalosporin group were 95.08%,91.80% and 96.72% respectively; the differenceshad no statistical significance(P〉0.05); the average cure time were(4.70 ± 1.92) d,(7.94 ± 4.31) d and(4.63 ± 1.66) d respectively.The penicillin group and the combination medication group were similar without statistical difference(P〉0.05),but the difference in the penicillin group was minimal(P〈0.05); the occurrence rates of adverse reactions were 11.48%,14.75% and 24.59% respectively,the difference between the penicillin group and the cephalosporins grouphad no statistical significance(P〉0.05),which was lower than that in the combined medication group,the difference was statistically significant(P〈0.05).Conclusion Single use of penicillin can obtain the better effect,time effect and lower level of intestinal flora influence in the treatment of neonatal infectious pneumonia,has less adverse reactions and is the first choice for treating this disease.
