摘要
目的:观察骨外洗方对减少手外伤术后患者皮肤疤痕增生的临床疗效。方法:选择2012年7月—2014年11月本院手外伤术后患者92例,随机分为对照组和治疗组各46例。对照组患者给予术后常规抗感染、手功能康复训练等基础治疗,治疗组在对照组治疗的基础上联合给予骨外洗方熏蒸外洗治疗,两组均连续治疗1个月。观察治疗前后患者疤痕痒痛的缓解情况及疤痕的软化平整情况,及疼痛缓解时间、创面愈合时间及疤痕面积;检测患者治疗前后的血液流变。结果:两组患者经治疗后疤痕痒痛及疤痕的软化平整程度均有所缓解,对照组临床治疗的有效率为71.7%,明显低于治疗组的有效率91.3%,差异有统计学意义(P<0.05);治疗组患者疼痛缓解时间、创面愈合时间较对照组明显缩短,疤痕面积较对照组明显减小,差异有统计学意义(P<0.05);患者经治疗后血液流变有所改善,血液流变学指标全血低切黏度值、全血高切黏度值、血小板聚集率、血浆比黏度、红细胞沉降率较治疗前有所降低,治疗组的降低幅度较对照组更为明显,差异有统计学意义(P<0.05)。结论:应用骨外洗方外洗能有效减少手外伤术后患者皮肤疤痕增生,缓解痛感,利于创面愈合,改善血液循环,缩短病程,疗效显著。
Objective： To observe the clinical effect of Bone External Lotion for reducing postoperative skin scar hyperplasia due to hand trauma. Methods： 92 postoperative patients of hand trauma received by this hospital from July,2012 to December,2014 were randomized into control group and treatment group,with 46 cases in each group. Control group were given postoperative conventional anti-infection and hand function rehabilitation training treatment. Treatment group were additionally given Bone External Lotion for fumigation and washing. Both groups were treated for one month to observe the relief of itching,pain,softening of the scar as well as the pain relief time,wound healing days and scar size before and after the treatment. The hemorheology before and after the treatment was also measured. Results： After the treatment,the itching,pain,softening of the scar all got relieved. The effective rate of control group was 71. 7%,obviously lower than tat of treatment group（91. 3%）; the difference had statistical significance（ P〈0. 05）. The pain relief time and wound healing days of treatment group were markedly shorten when compared with these of control group; the scar size in treatment group was also smaller than that in control group; the difference was statistically significant（ P〈0. 05）. The hemorheology was improved after treatment： the indexes of whole blood low shear viscosity,whole blood high shear viscosity,platelet aggregation rate,ratio of viscosity,and erythrocyte sedimentation rate all decreased after the treatment; and the decrease in treatment group was more obvious than that in control group,with statistical significance（P〈0. 05）. Conclusion： Self-made Bone External Lotion focuses on activating blood flow and relaxing the muscles and tendons.The clinical external washing with it can effectively reduce the postoperative skin scar hyperplasia due to hand trauma,relieve pain,facilitate the would healing,improve blood circulation,and shorten the disease process.
