摘要
目的:研究复方独正杆接骨汤对肱骨干闭合性骨折愈合过程中血清白细胞介素(IL)-1、IL-6、肿瘤坏死因子(TNF)-α水平的影响。方法:采用随机数字表法将64例肱骨干闭合性骨折患者分为治疗组与对照组,各32例。治疗组采用复方独正杆接骨汤内服治疗,对照组采用复方独正杆接骨膏外敷治疗,治疗60 d后,采用酶联免疫法测定两组患者血清IL-1、IL-6、TNF-α的变化水平,并统计两组患者总有效率。结果:治疗60 d后,治疗组与对照组总有效率差异无统计学意义(P>0.05);治疗组与对照组患者比较,治疗30 d和治疗60 d后血清IL-1、IL-6、TNF-α的水平差异无统计学意义(P>0.05),但各组与治疗前比较,差异具有统计学意义(P<0.05)。结论:复方独正杆接骨汤内服治疗肱骨干闭合性骨折与复方独正杆接骨膏疗效一致,这为复方独正杆接骨方的临床运用形式开辟了新的途径。复方独正杆接骨汤内服及复方独正杆接骨膏外用均能使患者血清IL-l、IL-6、TNF-α水平显著下降,减轻炎症反应,这可能是该方促进骨折愈合的机制之一。
OBJECTIVE:TO observe me ettect ot compouna auznenggan jiegu gecoctlon on me levels of IL-1, IL-6 ana TNF-a in the healing process of humeral shaft fracture. METHODS: 64 patients with humeral shaft fracture were randomly divided into treatment group (n= 32) and control group (n=32) by number. Treatment group was given Compound duzheaggan j iegu decoction and control group was given Compound duzhenggan jiegu cream. After 60 d of treatment, euzyme-linked immunosorbent assay was conducted to determine the change levels of serum IL-l, IL-6 and TNF-a and collect the total efficacy in 2 groups. RE- SULTS : After 60 d of treatment, there was no significant difference in the total efficacy between treatment group and control group (P〉0-05) ; compared with control group, there were no significant differences among the levels of serum IL-1, IL-6 and TNF-u in treatment group after 30 d and 60 d of treatment (P〉0-05) ; compared with before treatment and 30 d and 60 d of treatment, there was significant difference in the levels of serum IL-1, IL-6 and TNF-a in treatment group (P〈0-05), so as the control group (P〈 0-05). CONCLUSIONS: Compound duzhenggan jiegu decoction has the same efficacy with Compound duzhenggan jiegu cream in the treatment of humeral shaft fracture, which provides a new approach for the clinical application of Compound duzhenggan jiegu cream. Both Compound duzhenggan jiegu decoction and Compound duzhenggan jiegu cream can reduce the levels of serum IL-1, IL-6 and TNF-a and the inflammatory reactions by a mechanism that can promote fracture healing.
