老年冠心病患者冠状动脉介入治疗前阿托伐他汀联用曲美他嗪对肾功能的影响 被引量:23

Effect of combined trimetazidine and atorvastatin on renal function in elderly patients with coronary heart disease

摘要 目的观察老年患者冠状动脉介入治疗前阿托伐他汀联用曲美他嗪对肾功能的影响。方法连续筛选接受冠状动脉介入治疗老年患者136例,随机分为联合用药组66例和对照组70例;另根据高血压诊断标准筛选出合并高血压患者94例,又分为联合用药Ⅰ组42例和对照Ⅱ组52例。监测术前、术后24、48h血尿素、肌酐、估算的肾小球滤过率(eGFR)和胱抑素C。结果与术前比较,联合用药组术后24、48h尿素明显降低[(4.84±1.03)mmol/L、(4.65±1.23)mmol/L vs(5.55±1.38)mmol/L,P<0.01];与对照组比较,联合用药组术后48h胱抑素C明显降低(P<0.01)。与对照组Ⅱ比较,联合用药Ⅰ组术后48h肌酐、胱抑素C明显降低,eGFR明显升高(P<0.05,P<0.01)。结论老年患者冠状动脉介入治疗前短期服用阿托伐他汀联用曲美他嗪可保护肾功能,预防造影剂急性肾损害的发生,对于高血压患者具有同样效果。 Objective To study the effect of combined trimetazidine and atorvastatin on renal function in elderly patients with coronary heart disease(CHD).Methods One hundred and thirty-six elderly CHD patients who underwent coronary intervention therapy were randomly divided into combined treatment group(n=66)and control group(n=70).Another 94 hypertensive patients were divided into combined treatment group A(n=42)and control group B(n=52).Their serum urea,creatinine,Cys-C levels and eGFR were measured before and 24,48 hafter treatment.Results The serum urea level was significantly lower in combined treatment group 24 and 48hafter treatment than before treatment(P〈0.01).The serum Cys-C level was significantly lower in combined treatment group 48 hafter treatment than in control group(P〈0.01).The serum creatinine and Cys-C levels were significantly lower while the eGFR was significantly higher in combined treatment group A 48 hafter treatment than in control group B(P〈0.05,P〈0.01).Conclusion Short-term treatment with combined trimetazidine and atorvastatin can protect the renal function and prevent acute renal damage in elderly CHD patients before coronary intervention therapy and in hypertensive patients.
