经皮微创接骨板内固定技术联合锁定加压钢板治疗胫骨远端骨折的疗效分析 被引量:14

The observation about the clinical efficacy of combination LCP and MIPPO surgical for distal tibial fracture

摘要 目的分析经皮微创接骨板内固定技术(MIPPO)联合锁定加压钢板(LCP)治疗胫骨远端骨折的疗效。方法自2012年6月—2013年9月应用MIPPO联合LCP治疗60例胫骨远端骨折的患者。根据AO分类系统对胫腓骨远端骨折的标准分型,A型34例,B型23例,C型3例。对患者进行临床随访8-16个月,平均随访时间为12.3个月,随时记录胫骨远端骨折的愈合情况并定期复查X线片,统计术后患者出现并发症及损坏关节功能活动的情况。结果经该治疗后所有患者在术后骨折均达到骨性愈合。所有患者术后情况均根据Johner-Wruhs关节功能评定标准进行评定,其中术后4个月,35例为优,15例良,7例可,2例差,优良率为85.0%;术后8个月、12个月优良率分别为91.7%,96.7%。结论MIPPO联合LCP治疗胫骨远端骨折在该观察中获得满意的治疗效果。该方法具有固定牢靠、有助于骨折愈合、软组织等优点,符合生物学固定的理念,值得临床推广与应用。 Objective To observe the efficacy of surgical treatment of distal tibial facture with locking compression plate (LCP) combined with minimally invasive percutaneous plate osteosynthesis (MIPPO). Methods Form june 2012 to september 2013, 60 cases of distal tibial fractures were treated with LCP combined with MIPPO. According to AO classification,thirty-four patients be-longed to type A, twenty-three type B, and three type C.These patients were followed up from 8 months to 16 months with an aver-age time 12.3 months. X-ray re-inspected regularly,observed the stations of tibial fracture healing, and checked the postoperative complications and the activities of ankle function. Results All fracture reached bone healing. Press to Johner-Wruhs joint function assessment standards: 60 cases were followed up, 4 months postoperation excellent in 35, good in 15, fair in 7and poor in 2, and 85.0%excellent or good results were obtained;91.7%,96.6%excellent or good results were obtained in 8 months postoperation and 12 months postoperation. Conclusion Use of LCP combined with MIPPO obtained a satisfactory therapeutic effect. It provides re-liable and stable fixation , helps to fracture healing, is advantageous to the characteristics of soft tissue repair, is in accord with biological osteosynthesis (BO), worthy of clinical popularization and application.
