摘要
目的:检测同型半胱氨酸在非瓣膜性心房颤动患者中的表达水平,并探讨其临床意义。方法:入选2013年1月至2013年6月,我院心内科房颤中心的住院患者,其中阵发心房颤动患者220例,持续心房颤动患者168例。选取同期入院的阵发性室上性心动过速患者作为对照组(150例)。空腹采集静脉血,循环酶法按试剂盒标准流程检测血浆同型半胱氨酸的浓度。测量左心房内径(LAD)和左心室射血分数(LVEF)等心脏超声指标。结果:三组患者在心血管危险因素方面差异无统计学意义(P>0.05)。与对照组(10.9±2.2)μmol/L相比,心房颤动组的同型半胱氨酸浓度显著增高(P<0.05),且持续心房颤动组(15.2±3.0)μmol/L高于阵发心房颤动组(13.1±2.7)μmol/L,P<0.05。同型半胱氨酸与LAD之间呈现明显的正相关关系(r=0.36,P<0.01),与LVEF无相关关系(r=0.19,P=0.12)。结论 :心房颤动患者血浆同型半胱氨酸水平增高,且与LAD明显的正相关。
Objective: To examine the expression of homocysteine in nonvalvular atrial fibrillation patients and to explore its clinical significance. Methods: To enroll the in-patients in our Atrial Fibrillation Department from January 2013 to June 2013,including 220 paroxysmal atrial fibrillation patients and 168 persistent patients. 150 patients with paroxysmal supraventricular tachycardia were used as control. The fasting venous blood was collected on empty stomach at early morning,then the concentration of homocysteine was tested according to standard procedures of the diagnostic kit. The parameters of left atrial dimension and left ventricular ejection fraction were measured under echocardiography. Results: There were no significant differences in cardiovascular risk factors among three groups( P〉 0. 05). Compared to the control( 10. 9 ± 2. 2) μmol / L,the concentration of homocysteine was significantly higher in atrial fibrillation patients( P〈0. 05),furthermore,it was much more higher in persistent patients( 15. 2 ± 3. 0) μmol / L than in paroxysmal ones( 13. 1 ± 2. 7) μmol / L( P〈0. 05). There was a positive correlation between homocysteine levels and left atrial dimension( γ = 0. 36,P 0. 01),while no correlation between homocysteine levels and left ventricular ejection fraction was observed( γ = 0. 19,P = 0. 12). Conclusion: The expression of homocysteine was increased in atrial fibrillation patients,and it has a positive correlation with left atrial dimension.
