摘要
目的:探讨胎儿心脏复杂畸形的彩色多普勒超声表现和诊断价值.方法:从2013-01/2014-12我院实施彩色多普勒超声检查出的胎儿心脏复杂畸形患者中随机性抽取50例作为研究对象,对50例胎儿的影像学检查资料进行分析,总结胎儿心脏复杂畸形的超声表现.结果:研究结果显示,50例胎儿中,9例为心内膜垫缺损,7例为洛四联症,5例为大动脉转位,4例为永存动脉干,4例为单心室,3例为单心房,3例为肺动脉狭窄或闭锁,1例为单纯室间隔缺损,2例为主动脉弓离断,2例为主动脉弓离断缩窄,2例为误诊,8例为其他畸形.结论:四腔心切面、主动脉弓切面、大动脉短轴切面以及三血管-气管切面是彩色多普勒超声胎儿心脏复杂畸形最主要的切面,对于胎儿心脏复杂畸形的早期诊断具有重要价值,注重心房连接关系、肺动脉长轴切面和静脉、右室流出道、图像的缩放功能以及高频探头是避免诊断不全面和漏诊的重要因素.
Objective： To investigate the diagnostic value of color Doppler ultrasound on fetal cardiac malformation manifestations. Methods： 50 cases of patients was randomly selected as the research object ,who are diagnosed with fetal cardiac malformation by color Doppler ultrasonography in our hospital from January 2013 to December 2014,analyzing the imaging examination date of 50 cases fetal, and summarizing the sonographic findings of fetal cardiac complex malformation . Results： the results of the study show, 5, all of 50 cases fetuses ,9 cases were endocardial cushion defect, 7 cases were tetralogy of Fallot, 5 cases were transposition of the great arteries, 4 cases were persistent truncus arteriosus, 4 cases were single ventricle, 3 cases were single atrium, 3 cases were pulmonary stenosis or atresia, 1 cases was simple ventricular septal defect, 2 cases were interruption of aortic arch, 2 cases were interruption of aorta arch constriction, 2 cases were misdiagnosed, 8 cases of other malformation. Conclusion： the four chamber view, aortic arch view, great artery short axis view and three vessels and trachea view are Doppler color color Doppler ultrasonography of fetal cardiac malformation of the main section, which has important value for the early diagnosis of fetal cardiac malformation, pay attention to the relationship between atrial connections, long axis view of pulmonary artery and vein, right ventricular outflow tract, image zoom function and high frequency probe, is important factors for avoiding not complete diagnose and missed diagnose.
