摘要
目的探讨胆道再次手术的影响因素及预后效果。方法将新蔡县人民医院于2011年1月至2014年1月收治的452例胆道手术患者为研究对象。77例在首次胆道手术的基础上再行一次胆道手术的患者为实验组，375例单行一次胆道手术的患者为对照组。观察胆道再次手术的影响因素，实验组二次手术前后、对照组胆道手术患者的治疗效果评分与患者满意度评分。结果实验组二次手术前、手术后、对照组的治疗效果评分分别为（65．43±4．23）、（91．86±4．23）、（90．45±6．44）分，患者满意度评分分别为（79．42±7．55）、（94．23±3．53）、（92．14±4．21）分。实验组胆道二次手术前后两项评分比较差异均有统计学意义（P〈0．05）。实验组二次手术后与对照组两项评分比较差异未见统计学意义（P〉0．05）。结论肝胆外科手术中胆道再次手术的情况较为常见。对首次手术失败原因进行详细分析，选择正确的再次手术方法，可改善患者预后，提高胆道手术患者的满意度，值得临床推广使用。
Objective To investigate the influencing factors and prognosis of biliary reoperation. Methods From Jan 2011 to Jan 2014, 452 patients underwent biliary tract surgery in Xincai people＇ s hospital were selected as research subjects. Seventy-seven cases who re- ceived secondary biliary tract surgery as the experimental group, and the other 375 patients received biliary tract surgery as a single line of control group. The influencing factors of biliary reoperation, the effective scores and satisfaction scores of patients in the experimental group before and after the second surgery and the control group were observed and analyzed. Results The effective scores in experimental group before and after secondary surgery and control group, were （65.43 ± 4. 23 ）, （91.86 ± 4.23 ）, （90.45 ± 6.44） scores respectively, and the satisfaction scores were （ 79.42 ＋ 7.55 ）, （ 94. 23 ± 3.53 ）, （92. 14 ±4. 21 ）scores respectively. The two scores in experimental group before and after secondary biliary surgery were significantly different（ P 〈 0. 05 ）. And the ones between experimental group after the second surgery and the control group had no significant difference （ P 〉 0. 05）. Conclusions Biliary reoperation is common in hepatobiliary surgery. Detailed analysis of failure reasons of the first surgery, and choosing the correct way to reoperation can improve patients＇ outcomes, increase the satisfaction of biliary tract surgery, and it is worthy of clinical use.
