摘要
目的探讨一氧化氮合酶(iNOS)与基质金属蛋白酶-9(MMP-9)在胎膜早破中的表达及其临床意义。方法入选在我院进行规律产检的90例中足月胎膜早破者30例(tPROM组),早产胎膜早破者30例(pPROM组),正常足月妊娠并进行剖宫产终止妊娠者30例(对照组)。分别采用免疫组织化学染色和ELISA法检测胎膜、血清及羊水中iNOS和MMP-9的表达,并通过HE染色检测是否患有绒毛膜羊膜炎。结果 iNOS在pPROM组胎膜中的表达高于tPROM组(P<0.01)和对照组(P<0.01),在tPROM和对照组中表达无显著差异(P>0.05)。MMP-9在PROM组胎膜、血清及羊水中的表达均高于正常组,差异有统计学意义(P<0.01)。结论 iNOS和MMP-9的表达水平与PROM的发生密切相关,且iNOS和MMP-9的表达相互关联。
Objective To explore the clinical significance of matrix metalloproteinases-9 （MMP-9） and inducible nitric oxide synthase（iNOS） in patients with premature rupture of membranes and the relationship between MMP-9 and iNOS. Methods 60 pregnant women underwent PROM and 30 women delivered by cesarean section before labor were di- vided into three groups due to their state., the term premature rupture of the membranes group （tPROM）, the preterm premature rupture of the membranes group（pPROM）, and the control group who had cesarean section without PROM. Immuno histochemical method was used to detect the expression level of MMP-9 and iNOS in fetal membranes. The level of MMP-9 in serum and amniotie fluid was detected by ELLSA. Results The expression level of iNOS in fetal mem- branes in pPROM group was higher than that expressed in tPROM group（P〈0.01） and control group（P〈0.01）. A statistically significant difference of the expression of MMP-9 was also found in fetal membranes（P〈0.01）, serum（P〈 0.01） and amniotic fluid（P〈0.01） in PROM group than in control group. Conclusion The expression level of iNOS and MMP-9 has close relationship with PROM.
