摘要 目的了解肥胖与骨质疏松症(OP)的关系。方法东风公司医院用法国Medilnk公司全干式超声骨密度扫描仪,对健康检查者选择跟骨进行骨密度(BMD)监测。随机整群抽取东风公司十堰基地退休员工11180人的BMD监测资料,用描述流行学方法进行比较分析。结果 BMD下降率为41.8%,OP患病率为16.1%;〈50岁人群BMD下降率为26.5%,OP患病率为8.2%;〉50岁人群BMD下降率在41.6-43.5%,OP患病率在11.1-30.9%;男女BMD下降率分别为38.1%、44.7%,差异有统计学意义(χ^2=49.36,P〈0.01);OP患病率分别为11.7%、19.7%,差异有统计学意义(χ^2=130.92,P〈0.01)。体重指数阳性者BMD下降率(49.3%)明显高于阴性(36.9%)者,差异有统计学意义(χ^2=171.68,P〈0.01)。男性腰臀比值、超体重、体重指数阳性和阴性与OP患病的χ^2值在12.37-90.60之间,均P〈0.01。结论年龄的增长导致OP发生的危险性增高,女性发生OP的概率高于男性,体重指数阳性是BMD下降的危险因素,男性腰臀比值、超体重、体重指数阳性不是OP的危险因素。 [ Objective ] To investigate the relationship between obesity and osteoporosis (OP). [ Methods ] BMD of calcaneus was measured by France Medilnk Company made uhrasound BMD scanner for all individuals in Dongfeng Central Hospital. A total of 11 180 individuals were included by random cluster sampling from Dongfeng Motor Corporation retired staff in Shiyan. The descriptive epidemiology was adopted for comparative analysis.[ Results] The rate of decline in BMD was 41.8%, the prevalence rate of OP was 16.1%. In 〈 50 years age group, the rate of decline in BMD was 26.5%, the prevalence rate of OP was 8.2%; In 〉50 years age group, the rate of decline in BMD was 41.6%-43.5%, the prevalence rate of OP was 1i.1%-30.9%. The rate of decline in BMD of male and female was 38.1%, 44.7% respectively, with significant difference( χ^2=49.36, P〈0.01 ); the prevalence rate of OP was 11.7%, 19.7% respectively, (χ^2=130.92, P〈0.01 ). Compared with lower BMI group (36.9%), the rate of decline in BMD increased significantly in higher BMI group (49.3%), with significant difference( χ^2=171.68, P〈0.01 ). The χ^2 value of male waist-hip ratio, overweight, lower and higher BMI with the prevalence rate of OP was between 12.37-90.60, all P〈0.01. [ Conclusion ] The increasing age causes the increasing risk of OP, the prevalence of OP in female is higher than male, higher BMI is a risk factor of BMD declined, male waist-hip ratio, overweight and higher BMI are not associated with risk of OP.
