摘要
目的:探讨新生儿颅内出血患儿血浆基质金属蛋白酶-2(MMP-2)、基质金属蛋白酶-9(MMP-9)、C-型利钠肽(CNP)含量及其与神经功能缺损、出血量的关系。方法:选择本院新生儿科颅内出血患儿72例作为观察组,将72例患儿按照神经功能缺损将其分为轻型组26例、中型组29例、重型组17例,按照出血量分为小量组25例、中量组31例、大量组16例,分析颅内出血患儿神经功能缺损评分、出血量与患儿血浆MMP-2、MMP-9、CNP含量的关系。结果:观察组血浆MMP-2、MMP-9以及CNP水平均较对照组儿童明显升高,差异具有统计学意义(P<0.05);不同神经功能缺损程度患儿血浆MMP-2、MMP-9以及CNP水平与对照组比较,均明显升高,且重型组>中型组>轻型组,差异具有统计学意义(P<0.05);不同出血量患儿血浆MMP-2、MMP-9以及CNP水平与对照组比较,均明显升高,且大量组>中量组>小量组,差异均具有统计学意义(P<0.05);颅内出血患儿其神经功能缺损评分与血浆MMP-2、MMP-9以及CNP水平均呈正相关关系(P<0.05);颅内出血患儿出血量与血浆MMP-2、MMP-9以及CNP水平均呈正相关关系(P<0.05)。结论:新生儿颅内出血患儿其血浆MMP-2、MMP-9以及CNP水平高于健康新生儿,并且血浆MMP-2、MMP-9以及CNP水平与患儿神经功能缺损、出血量密切相关,可作为判断患儿病情的相关参考指标。
Objective: To investigate relationship between plasma MMP-2, MMP-9, CNP content and defect of nerve function, amount of bleeding of patients with neonatal intracranial hemorrhage. Methods: A total of 72 patients with neonatal intracranial hemorrhage were selected as the observation group, and were divided into were divided into mild group (n= 26), medium group (n = 29) and severe group (n = 17) in according to neural function defect ; and divided into mild group (n = 25), medium group (n= 31) and severe group (n=16) according to the amount of bleeding. Relationship between plasma MMP-2, MMP-9, CNP content and defect of nerve function, amount of bleeding was analyzed. Results: In the observation group plasma MMP 2, MMP-9 and CNP levels were significantly increased compared with the control group (P〈0.05). Compared with the control group different defect degree of the nervous function in children with plasma MMP-2, MMP-9 and CNP levels were sig-nificantly increased (P〈0. 05), and severe group 〉 medium group 〉 light group, with statistically significant difference between the groups (P〈0.05). Compared with the control group different amount of bleeding with plasma MMP-2, MMP-9 and CNP levels were significantly increased (P〈0. 05), as large amount groups〉medium amount group〉 small amount group, with significant difference between groups (P〈0.05). There was significantly positive correlation between the nerve in chil- dren function defect score and plasma MMP-2, MMP-9 and CNP level of intracranial hemorrhage (P〈0. 05); intracranial hemorrhage volume were significantly positively correlated with plasma MMP-2, MMP-9 and CNP levels (P〈0.05). Conolusions: The plasma levels of MMP-2, MMP-9 and CNP level of neonatal intracranial hemorrhage are increased, and is closely related with neurological functional defect, and the amount of bleeding. It can be used to judge the relevant indexes in children with the disease.
