摘要 目的研究电刺激联合生物反馈治疗对产后盆底功能的恢复作用,拟建立一个客观评估体系明确刺激联合生物反馈治疗的目标。方法回顾性分析2013年7月至2014年6月在湖北省监利县人民医院行电刺激生物反馈治疗的患者,根据治疗效果问卷分为治疗满意组、一般组、不满意组,比较治疗前后及组间肌力、盆底肌张力及肌电位值,用SPSS 19.0分析。结果符合要求的患者共计181例,其中治疗满意组92例,一般组50例,不满意组39例。经过电刺激联合生物反馈治疗后,三组患者肌力等级、阴道动态压、肌电位值均有不同程度的升高,其中治疗效果满意组升高最快,一般组次之,治疗效果满意组与其他两组比较差异有显著性。结论电刺激联合生物反馈治疗用于产后恢复盆底功能是有效的,为提高治疗效果建议给患者拟定一个合适的治疗目标,建议训练Ⅰ类、Ⅱ类肌纤维肌力,阴道动态压力提升约30 cm H2O,Ⅰ类肌纤维的肌电值增加为治疗前的3倍,Ⅱ类肌纤维的肌电值增加为治疗前的2.5倍。 Objective To establish a evaluation system which was used to evaluate the effect of electrical stimulation combined biological feedback for pelvic floor function after postpartum, and improve the effect of treatment. Methods Retrospectively analyzed patients who accepted electrical stimulation combined biological feedback treatment in Jianli Renmin Hospital of Hubei province from July 2013 to June 2014. The patients were divided into three group according to whether they were satisfied with the treatment effect. We compared the basic imformation/muscle strength grade, vaginal pressure level/the voltage of muscle fiber, SPSS 19.0 was used for statistics. Results There were 181 patients who met our requirements in which 92 in satisfactory therapeutic effect group, while 50 in normal group, 39 in unsatisfactory therapeutic effect group. After one course electrical stimulation combined biological feedback therapy, the indicators including muscle strength grade, vaginal pressure level/the voltage of muscle fiber were improved especially in satisfactory therapeutic effect group, the second was normal group. Conclusions Electrical stimulation combined biological feedback for pelvic floor function after postpartum is effective, in order to improve the treatment effect, training pelvic floor muscle strength is suggested, we should increase vaginal dynamic pressure about 30 emH20, the voltage of muscle fiber I is 3 times as many as that of before treatment, and the voltage of muscle fiber Ⅱ promotes to 2.5 times that of before treatment. Patients in satisfactory therapeutic effect group had obvious advantage compared with the other two.
