摘要
目的探讨翼状胬肉切除术后并发眼球筋膜肉芽肿的原因、临床表现及治疗方法。方法回顾性分析21例（21眼）翼状胬肉切除术后并发筋膜肉芽肿的临床特点，治疗组11例行筋膜肉芽肿切除＋自体结膜瓣移植术，对照组10例行筋膜肉芽肿切除术未行结膜移植，其余治疗方法同治疗组。对两组资料进行临床分析。结果21例切除的筋膜肉芽肿病理切片检查为炎性肉芽组织，细菌培养结果提示未见细菌生长。治疗组11例未见筋膜肉芽肿复发，随访12个月未见翼状胬肉复发；对照组4例术后2～3周筋膜肉芽肿复发，再次手术行肉芽肿切除＋自体结膜瓣移植术，6例术后第3周结膜创口仍轻度充血、增厚，随访12个月，筋膜肉芽肿未见复发，但4例翼状胬肉复发。结论筋膜肉芽肿切除＋自体结膜瓣移植术治疗翼状胬肉切除术后并发筋膜肉芽肿是安全有效的，而且取材方便，并可降低翼状胬肉复发率。
Objective To investigate the causes, clinical manifestation and treatment for fascial granuloma after excision of pterygium. Methods Twenty-one eyes of 21 cases with fascial granuloma after excision of pterygium were enrolled in the study. The cases were divided into two groups ： 11 eyes of 11 cases received granuloma excision and conjunctival autografting （treatment group） and 10 eyes of 10 cases received granuloma excision only （ control group）. The data of two groups were analyzed. Results The biopsy result of the excisional granuloma in 21 cases were inflammatory granulation tissue, and no bacteria grew in bacterial culture. Patients were followed up for 12 months. There was no recurrence in the treatment group. Granuloma relapsed in 4 eyes of 4 cases in the control group, and finally the recurrent cases received granuloma excision and eonjunctival autografting. The conjunctival wounds in 6 eyes of 6 cases were mild congestive and incrassated 3 weeks after the operation. After following up for 12 months, no granuloma recurred but pterygium recurred in 4 eyes of 4 cases in control group. Conclusion Fascial granuloma excision with conjunctival autografting is a safe, effective and easy method in treating fascial granuloma after excision of pterygium, and can decrease the recurrence of pterygium.
