摘要 目的探讨胰岛素样生长因子(IGF)在老年糖尿病患者血清中的表达及其与骨质疏松的关系。方法根据骨密度(BMD)将2型糖尿病(T2DM)患者98例分为骨量正常组30例、骨量减少组37例、骨质疏松组31例,检测各组体质量指数(BMI)、空腹血糖(FBG)、餐后2 h血糖(PPG)、糖化血红蛋白(Hb Alc)、骨碱性磷酸酶(BALP)、血清N-端骨钙素(N-MID)及IGF-1、IGF-2浓度,并分析上述各指标与BMD的相关性。结果骨量减少组和骨质疏松组BMI低于骨量正常组(P<0.01);N-MID、IGF-1、IGF-2水平在骨量正常组、骨量减少组及骨质疏松组之间均呈逐步下降趋势,两两比较,N-MID、IGF-1水平差异均有统计学意义(P<0.05,P<0.01);相关性分析表明,BMD与IGF-1、N-MID、BALP均呈正相关(r=0.672,0.624,0504,P<0.01)。结论血清IGF-1与老年T2DM患者骨代谢相关指标密切相关,可能参与了骨质疏松的发生发展。 Objective To explore the expression of insulin-like growth factor( IGF) in the serum of elderly patients with diabetes and its relationship with osteoporosis. Methods A total of 98 patients with type 2 diabetes mellitus( T2DM) were divided into normal bone mass group( n = 30),reduced bone mass group( n = 37) and osteoporosis group( n = 31) according to the bone mineral density( BMD). Fasting blood glucose( FBG),2-h postprandial blood glucose( PPG),glycosylated hemoglobin( Hb Alc),bone alkaline phosphatase( BALP),N-MID osteocalcin( OT) as well as the concentrations of IGF-1 and IGF-2 were detected and compared among all the groups. Results The BMI in the reduced bone mass group and the osteoporosis group were evidently lower than the normal bone mass group( P〈0. 01). N-MID,IGF-1 and IGF-2 levels showed a decreasing trend in the normal bone mass group,the reduced bone mass group and the osteoporosis group in sequence,and pair wise comparison indicated that there were significant differences in N-MID and IGF-1 levels( P〈0.05 or P〈0. 01). Correlation analysis demonstrated that BMD was positively associated with IGF-1,N-MID and BALP( r = 0. 672,0. 624,0504,P〈0. 01). Conclusion Serum IGF-1 is closely associated with bone metabolism-related indexes,and it may participate in the development and progression of osteoporosis.
