摘要
目的研究CD4+、CD8+、CD8+CD28-Treg细胞在过敏性紫癜中的表达及临床意义。方法 78例确诊的过敏性紫癜患者在入院时各采静脉血2 ml,予肝素抗凝后行流式细胞术(flow cytometry,FCM)检测CD4+、CD8+、CD8+CD28-Treg细胞的表达水平。78例患者给予抗组胺药物、维生素C、芦丁片、钙剂治疗,对于紫癜型肾炎组病情严重者给予丙球蛋白治疗以缓解症状;治疗2周后治愈或好转后再次采取外周血样,流式细胞仪检测外周血CD4+、CD8+、CD8+CD28-Treg细胞的表达水平。结果治疗后患者外周血中CD4+上升、CD8+下降、CD8+CD28-Treg细胞上升明显。结论外周血CD8+CD28-Treg细胞的表达水平对过敏性紫癜的诊断和治疗具有一定的指导作用。
Objective To research expression and clinical value of CD4^＋ T cell,CD8^＋ T cell,CD8^＋ CD28^- Treg cell in patients with allergic purpura.Methods 2ml peripheral blood was drawn from 78 allergic purpura patients respectively. Heparin was added to the drawn peripheral blood. CD4^＋ T cell, CD8^＋ T cell, CD8^＋ CD28^- Treg cell in the blood were detected by flow cytometry （FCM）. These patients were treated with Histamine drugs, Vitamin C, Rutin tablets and Calcium supplements. In order to relieve symptoms and hemorrhage, sugar cortical hormone and immune globulin were used to treat patients with severe kidney disease symptoms.Two weeks later, peripheral blood sample was collected again and the expression levels of CD4^＋ T cell, CD8^＋ T cell and CD8^＋ CD28^- Treg cell in the peripheral blood were detected by FCM.Result After allergic purpurapatients were treated, CD4^＋ cells in peripheral blood increased, and CD8^＋ T cells in peripheral blood decreased. CD8^＋ CD28^- Treg cells were obviously increased.Conclusion CD8^＋ CD28^- Treg cells in peripheral blood have instructive meaning for the diagnosis and treatment of allergic purpura.
