摘要
目的探讨早期应用参附注射液对老年重症肺炎患者血浆脑钠肽(BNP)水平的影响。方法选择2012年4月—2014年4月河北联合大学附属医院综合呼吸重症监护室收治的老年重症肺炎患者80例,按照随机数字表法分为对照组和参附注射液组,各40例。对照组患者给予西医综合治疗,参附注射液组患者在西医综合治疗的基础上给予参附注射液治疗。入院当天及治疗后1 d、3 d、5 d检测患者血浆BNP水平;入院当天及治疗后3、5 d采用超声心动图对患者进行检查,采用急性生理学及慢性健康状况评分系统Ⅱ(APACHEⅡ)对患者进行评分;并评定治疗后7 d患者的临床疗效。结果治疗7 d后对照组患者临床疗效优于对照组(P<0.05)。两组患者入院当天和治疗后1d血浆BNP水平比较,差异无统计学意义(P>0.05);参附注射液组患者治疗后3 d和5 d血浆BNP水平低于对照组(P<0.05)。两组患者入院当天APACHEⅡ评分、每搏输出量(SV)、心输出量(CO)、心脏排血指数(CI)及左心室射血分数(LVEF)比较,差异无统计学意义(P>0.05);参附注射液组患者治疗后3 d和5 d APACHEⅡ评分低于对照组,SV、CO、CI及LVEF高于对照组(P<0.05)。结论早期应用参附注射液能有效降低老年重症肺炎患者血浆BNP水平,促进患者心功能恢复。
Objective To investigate the influence of shenfu injection on plasma BNP level of early aged severe pneumonia. Methods From April 2012 to April 2014,80 aged patients with severe pneumonia at early stage were selected in the Comprehensive Respiratory ICU of the Affiliated Hospital of Hebei United University, and they were divided into groups control and observation according to random number table, each of 40 cases. Both groups were given Western medicine comprehensive treatment,while observation group was given extra shenfu injection. Plasma BNP level was detected at admission,1 day,3 days,5 days after treatment; ultrasonic cardiogram was used to examine SV,CO,CI and LVEF,APACHEⅡ score was evaluated; clinical effect was observed after 7 days of treatment. Results The clinical effect of observation group was better than that of control group( P〈0. 05). No statistically significant differences of plasma BNP level was found between the two groups at admission,1 day after treatment,respectively( P〈0. 05); plasma BNP level of observation group was lower than that of control group 3 days,5 days after treatment,respectively( P〈0. 05). No statistically significant differences of SV,CO,CI,LVEF and APACHE Ⅱ score was found between the two groups at admission( P〈0. 05); while APACHE Ⅱ score of observation group was lower than that of control group 3 days,5 days after treatment,respectively( P〈0. 05),SV,CO,CI and LVEF of observation group were higher( P〈0. 05). Conclusion Shenfu injection can effectively reduce the plasma BNP level of aged patients with severe pneumonia at early stage,improve the cardiac function.
