摘要
目的探讨成人腺病毒肺炎的高分辨CT(HRCT)表现及动态变化。方法收集某医院2011年12月-2014年3月确诊的成人腺病毒肺炎患者47例。于发病第3、7、15、30、45及60天分别行双源HRCT扫描,回顾性分析其CT资料并文献复习。结果病情轻微者CT显示肺纹理增多、肺叶或肺段小片状或结节样密度增高影,吸收快并且完全。重症者表现为单个或多个肺叶实变影伴支气管充气征、局限胸膜增厚及胸腔积液,吸收较慢,病情稳定后表现为肺叶条片状稍高密度,相应小叶间隔增厚,相邻胸膜增厚、粘连。临床治愈出院时及近期复查时绝大部分病灶消失,个别表现为病变区砂粒样钙化或胸膜粘连。结论 HRCT动态观察能较客观的反映本病从早期到恢复期的病理改变过程,可指导临床治疗并评估预后。
Objective To study the chest high resolution computed tomography（HRCT） manifestation and dynamic changes in adult patients with adenovirus type 7 pneumonia.Methods Forty-seven male patients underwent CT scan and diagnosed with adenovirus pneumonia in our hospital from December 2011 to March 2014 were reviewed.Dual energy HRCT scans were performed at day3,7,15,30,45 and 60 of disease onset.CT images were retrospectively reviewed together with journal related articles.Results At early stage of disease with slight symptoms,CT showed increased lung markings,small schistose of pulmonary lobes or segments,and nodules of increased density.Absorbing was quickly and completely.Severe patients,showing single or multiple lobe consolidation,with air bronchogram,localized pleural thickening and pleural effusion.Those patients with slow absorption showed lobes schistose with higher density,thickening of corresponding interiobular septal,adjacent pleural thickening and adhesions in stable condition.After success clinical treatment,most lesions were disappeared,and only a few individuals showed sand-like calcification or pleural adhesions.Conclusion CT scans of early adult adenovirus pneumonia show single or multiple schistose and nodular pulmonary parenchymal inflammations.Disease then gradually developed into parenchymal and interstitial lesions,showing as pulmonary lobe or segment consolidation.During the recovery stage,interstitial lung diseases are the majorities.Through dynamic observation on HRCT data,the physiology changes of the disease have been revealed clearly and objectively,which can be valuable to clinical treatment and prognosis prediction.
