摘要
目的:探讨高频超声与X线钼靶联合诊断乳腺浸润性导管癌的价值。方法:对经病理证实的乳腺浸润性导管癌患者60例,肿块总数为62例,分析其超声及X线表现并作出诊断,探讨联合诊断的价值。结果:超声显示肿块阴影58例(93.5%),实性57例(91.9%),囊实性1例(1.6%),单侧56例(90.3%),双侧2例(3.2%),实性结节均呈低回声,形态不规则,无包膜,呈蟹足样28例(45.1%),纵横比>1 15例(24.2%),内部回声不均匀54例(87.1%),后方伴声衰减9例(14.5%),钙化影24例(38.7%),41例血流丰富(66.1%),腋窝淋巴结转移16例(25.8%)。超声准确率83.9%,误诊率11.2%;钼靶摄影诊断准确率79.0%,误诊率14.5%;两种方法联合,准确率90.3%,误诊率6.5%。结论:高频超声与X线钼靶对乳腺浸润性导管癌诊断有很多帮助,结合应用可明显提高浸润性导管癌的诊断准确率。
[Abstract] Objective： To explore the high frequency ultrasound and mammography in the diagnosis of breastinvasiveductal cancer.Methods： pathologically confirmed breast invasive ductal carcinoma in 60 cases, mass for a total of 62 cases, analysis of the ultrasound and X-ray findings and diagnosis, the diagnostic value of combined. Results： the ultrasound showed mass shadow in 58 cases （93.5%）, solid in 57 cases （91.9%）, 1 cases of cystic and solid （1.6%）, 56 cases （90.3%）, 2 cases of bilateral （3.2%）, solid nodules were hypoechoic, irregular shape, no envelope, a crab like （28 cases 45.1%）, aspect ratio more than 115 cases （24.2%）, inhomogeneous echo in 54 cases （87.1%）, 9 cases with posterior acoustic attenuation （14.5%）, calcification in 24 cases （38.7%）, 41 cases with rich blood flow （66.1%）, axillary lymph node metastasis in 16 cases （25.8%）. The accuracy of ultrasonography was 83.9%, the misdiagnosis rate was 11.2%; mammography diagnosis accurate rate was 79%, the misdiagnosis rate was 14.5%; the combination of the two methods, the accuracy rate was 90.3%, the misdiagnosis rate was 6.5%. Conclusion： high frequency ultrasound and mammography in the diagnosis of a lot of help in invasive ductal carcinoma of the breast, and combined with the application, can significantly improve the diagnostic accuracy rate ofinvasive ductal carcinoma.
