摘要
目的探索宝石能谱成像(GSI)评价痛风患者双膝关节尿酸盐沉积规律的临床价值。方法对32例痛风患者行双膝关节GSI扫描,分析双膝尿酸盐沉积分布规律,同时统计分析尿酸盐与周围各组织定量分析指标:有效原子序数(Eff-Z)、能谱曲线斜率、尿酸(钙)[UA(ca)]含量、钙(尿酸)[CA(ua)]含量及UA(ca)/CA(ua)值。结果 32例患者均检测到尿酸盐,分布于213处沉积区,且内外侧副韧带处(74/213,21.13%)>交叉韧带(35/213,16.43%)>股四头肌肌腱(18/213,8.45%)。尿酸盐Eff-Z、曲线斜率、UA(ca)含量、CA(ua)含量和UA(ca)/CA(ua)值分别为8.10±0.26、1.06±0.55、(1260.30±50.65)mg/cm3、(18.93±6.26)mg/cm3和68.31±24.96,与周围骨皮质、肌肉、骨松质比较差异均有统计学意义(P均<0.05)。结论尿酸盐在膝关节周围易沉积于受力较大的肌腱及韧带处。GSI多参数分析有助于尿酸盐的定性诊断,UA(ca)/CA(ua)值对尿酸含量变化可能更敏感。
Objective To investigate the clinical value of gemstone spectral imaging(GSI)in evaluating features of uric acid salt's deposition in joint of knee.Methods Gemstone spectral CT scans of knee were performed on 32 patients with gout.The features of uric acid salt's deposition were investigated;spectrum parameters of uric acid salt and tissues around,including effective atomic number(Eff-Z),spectrum curve slope,content of uric acid(calcium)—(UA [ca]),content of calcium(uric acid)—(CA [ua]),specific value of UA(ca)/CA(ua)were statistically analyzed.Results The deposition of uric acid salt was found in all patients,which distributes in 213 areas.The most common affected sites were medial and lateral collateral ligaments(74/210,21.13%),cruciate ligament(35/210,16.43%),quadriceps tendon(18/213,8.45%)successively.The Eff-Z,spectrum curve slope,UA(ca),CA(ua)and UA(ca)/CA(ua)of uric acid salt were8.10±0.26,1.06±0.55,(1260.30±50.65)mg/cm3,(18.93±6.26)mg/cm3 and 68.31±24.96,which were significantly different with cortical bone,muscle,cancellous bone around.Conclusion The uric acid salt deposites more in ligaments and tendons of knee where stresses more.The GSI parameter analysis can help qualitatively diagnosis of uric acid salt,UA(ca)/CA(ua)may be more sensitive to detect the change of uric acid.
