减压手术治疗急性脊髓损伤的研究进展 被引量:2

Recent advance in decompressive surgery in treating acute spinal cord injury

摘要 急性脊髓损伤(acute spinal cord injury, ASCI)在世界范围的发病率为15~40/百万人。可引起患者严重的神经功能障碍.并增加整个家庭及社会的经济负担[1]。ASCI好发于年轻、健康人群,好发年龄为25~35岁,男女比例为4:1[1]。据计算,一个25岁颈部ASCI致四肢瘫的患者,在其有生之年。医疗及其他相关费用大约为300万美元[2]。ASCI常见的原因有:机动车事故(50%)、坠落和职业相关的损伤(30%)、暴力犯罪(11%)、运动损伤(9%)[2]。脊柱活动度大的部位较容易受到损伤,ASCI最常见于颈椎,来自美国国家脊髓损伤数据库的资料显示,56%的ASCI发生在颈椎[3]。本文就ASCI的病理生理变化、硬膜外减压与硬膜内减压手术治疗ASCI的研究进展做一综述。
