摘要
目的：通过比较悬吊式免气腹与气腹腹腔镜胃癌根治术的差异，探讨悬吊式免气腹腹腔镜下胃癌根治性的可行性及应用前景。方法选取潍坊市人民医院自2012年10月～2014年5月施行的悬吊式免气腹腹腔镜下胃癌根治性切除术31例，其中根治性远端胃切除术15例，根治性近端胃切除术9例，根治性全胃切除术7例，与随机选取同期完成的气腹腹腔镜胃癌根治术（相同分期）30例资料作回顾性分析。结果61例患者的手术均获成功，无中转开腹及术中死亡的情况发生。免气腹组术后肛门首次排气时间、术后并发症发生率优于气腹组（ P＜0．05）。结论免气腹腹腔镜用于胃癌根治术安全可行，能够达到根治要求，兼具有气腹腹腔镜的优点，比之更有并发症少且术后康复快的优势，具有推广价值。
[ ABSTRACT] Objective To compare the differences between under suspended gasless laparoscopic surgery and pneumoperitoneum laparoscopic surgery in gastric cancer gasless laparoscopic gastrectomy and laparoscopic gastrectomy gas ,and to explore the feasibility and ap-plication prospects of suspended Gasless Laparoscopic Surgerygastric radical under gasless laparoscopy .Methods Thirty-one patients with suspended Gasless Laparoscopic Surgery of gastric cancer in Weifang People ＇s Hospital from October 2012 to May 2014 were collected ,among them,15 cases with radical distal gastrectomy ,9 cases with radical proximal gastrectomy and 7 cases with radical total gastrectomy .A retro-spective analysis between data above and another 30 cases with gasless laparoscopic gastric cancer （in the same stage） radical prostatectomy at the same term was made.Results All of the operations were successful without turning to open .61 cases of patients were successful,no conversion to open surgery cases and deaths occured;surgery time gasless group＇s first anal exhaust time and postoperative complication rate was lower than that of pneumoperitoneum group （P〈0.05）.Conclusion Gasless laparoscopic radical gastrectomy is safe and feasible ,be-sides,becauso of its achieved radical demands and also has the advantages of laparoscopic insufflationinsufflations ,it is worthy of populari-zing.
