脑室-腹腔分流术后重度低钠血症的原因分析及治疗体会

摘要 目的分析脑积水患者经脑室-腹腔分流术后低钠血症的原因,探讨其治疗的方法。方法选取有临床症状及有影像学表现的患者为研究对象,行脑室-腹腔分流术,术后检查患者电解质。低于120 mmol/L患者考虑为重度低钠血症。结果一例患者通过单纯补钠治愈;一例患者通过补钠和口服强的松治愈;三例患者通过补钠、口服强的松脑室和调高分流管压力后治愈。结论所有低钠血症患者经临床证实均为脑性耗盐综合征;下丘脑等部位对颅内压的突然改变的不适应,可能是导致低钠血症的发生的原因。 Objective To analyze the etiology of hyponatremia in patients with hydrocephalus after ventriculoperitoneal shunt( V-P shunt) and to investigate the therapeutic method. Methods The patients who had clinical symptoms and imaging manifestations of the hydrocephalus were selected as subjects and received V-P shunt. The electrolytes were measured after V-P shunt and the patients whose serum sodium concentrations were below 120 mmol / L were considered with serve hyponatremia. Results One patient was cured by simple salt supplement,one patient was cured by salt supplement and oral prednisone,and three patients were cured by salt supplement,oral prednisone,and increasing the pressure of catheters. Conclusions All patients with hyponatremia are confirmed as having cerebral salt wasting syndrome. The etiology of hyponatremia may be that the hypothalamus or other sites cannot adapt to the sudden change in intracranial pressure.
